摘要
目的分析肝衰竭患者并发感染的危险因素及感染对肝衰竭患者近期预后的影响。方法收集蚌埠医学院第一附属医院收治的278例肝衰竭患者的临床和实验室资料,根据感染情况分为感染组和非感染组,分析两组的临床和实验室指标及预后情况。结果 278例肝衰竭患者中,并发感染121例,感染率43.53%。多发感染17例,占感染总例数的14.05%。Logistic回归分析显示住院时间、血浆白蛋白、终末期肝病模型评分(MELD评分)、腹水、肝性脑病及低钠血症是诱发肝衰竭患者感染的危险因素。感染组住院期间病死率(12.40%)明显高于非感染组(5.10%)。感染部位越多,患者的近期预后越差。结论住院时间、血浆白蛋白、腹水、肝性脑病、低钠血症是诱发肝衰竭患者发生感染的危险因素,合并感染的肝衰竭患者预后更差。
Objective To analyze the risk factors of infection in patients with liver failure and the effect of infection on the short-term prognosis of patients with liver failure. Methods The clinical and laboratory data of278 patients with liver failure in the First Affiliated Hospital of Bengbu Medical College were collected. According to the infection,the patients with liver failure were divided into infection group and non-infection group,and the clinical and laboratory indexes and prognosis of the two groups were analyzed. Results Among 278 patients with liver failure,121 cases were complicated with infection,the infection rate was 43.53%. Multiple infections were found in 17 cases,accounting for 14. 05% of the total infections. Logistic regression analysis showed that hospitalization time, plasma albumin, end-stage liver disease score(MELD score), ascites, hepatic encephalopathy and hyponatremia were the risk factors of infections in patients with liver failure. The fatality rate in the infection group(12.40%) was significantly higher than that in the non-infection group(5.10%). The liver failure patients with increased number of infection sites had poorer recovery on the short-term prognosis.Conclusions Hospitalization time,plasma albumin,ascites,hepatic encephalopathy,hyponatremia are risk factors for infection in patients with liver failure. It is more serious for the prognosis of patients with liver failure complicated by infection.
