摘要
目的观察不同剂量甲磺酸倍他司汀片治疗良性发作性位置性眩晕(BPPV)患者复位治疗后残余头晕的疗效。方法将行管石复位术成功的BPPV残余头晕患者150例,随机分为3组,对照组(n=50)手法复位成功后未予甲磺酸倍他司汀片治疗,治疗1组(n=50)在手法复位成功后口服甲磺酸倍他司汀片6 mg/次,3次/d,治疗4周;治疗2组(n=50)在手法复位成功后口服甲磺酸倍他司汀片12 mg/次,3次/d,治疗4周。比较3组患者残余头晕症状的消失时间和3组患者治疗前以及治疗第1、2、4周时的眩晕障碍量表(DHI)分数。结果治疗后治疗1组及治疗2组均能缩短残余头晕症状的时间(P<0.05),但治疗后第1、2、4周时治疗1组和对照组的DHI评分无明显差异(P>0.05);治疗2组在治疗第1、2周时评分和对照组亦无明显差异(P>0.05),在第4周时分值明显下降,差异有统计学意义(P<0.05)。结论甲磺酸倍他司汀治疗应足量足疗程使用才能对BPPV患者复位治疗后残余头晕有治疗作用。
Objective To observe the effect of different doses of betahistine mesylate on residual dizziness after treatment of benign paroxysmal positional vertigo(BPPV）. Methods 150 patients with residual dizziness after successful restoration of BPPV was randomly divided into three groups： Control group(n = 50） did not use betahistine mesylate after successful restoration,first treatment group(n = 50） used the oral betahistine mesylate 6 mg/time,3 times/d,for 4 weeks after successful restoration,second treatment group(n = 50） used the oral betahistine mesylate 12 mg/time,3 times/d,for 4 weeks after successful restoration. Analysed the duration of residual dizziness,the score of Dizziness Handicap Inventory(DHI） of residual dizziness before treatment,1 week,2 weeks,4 weeks after treatment. Results The first and second treatment groups had shorter time of residual dizziness(P〉0. 05）. The differences of the score of DHI before treatment,1 week,2 weeks,4 weeks after treatment between control group and first treatment group had no significance(P〉0. 05）. The differences of the score of DHI before treatment,1 week,2 weeks after treatment between control group and second treatment group had no significance(P〉0. 05）,but the differences of the score of DHI in 4 weeks between control group and second treatment group had significance(P〈0. 05）. Conclusion The treatment of patients with BPPV can be treated with a full course of treatment of betahistine mesylate.
