摘要
目的探讨标准大骨瓣开颅血肿清除术治疗重型颅脑外伤侧裂区损伤的效果及对患者生存率的影响。方法选取重型颅脑外伤侧裂区损伤患者96例，按照手术方法分为对照组与观察组，每组48例，对照组行常规大骨瓣开颅血肿清除术，观察组行标准大骨瓣开颅血肿清除术。比较两组手术前后中线移位、格拉斯哥昏迷指数（GCS）评分、生存率以及并发症发生率。结果观察组患者GCS评分明显高于对照组，中线移位、生存率及并发症发生率明显低于对照组（P〈0．05）。结论标准大骨瓣开颅血肿清除术治疗重型颅脑外伤侧裂区损伤效果较好，可有效提高患者GCS评分及远期生存率，且并发症较少，有助于提高患者生存质量，值得临床推广应用。
Objective To investigate the effect of standard large-bone craniectomy for hematoma evacuation on severe craniocerebral injury in lateral fissure area and its influence on the survival rate. Methods Ninety-six patients with severe craniocerebral trauma in lateral fissure area were divided into observation group and control group according to operation methods, with 48 cases in each group. Pa-tients in control group were treated by conventional large-bone craniotomy and hematoma evacuation, while patients in observation group were treated by standard large-bone craniotomy and hematoma evacua-tion. The midline shift, Glasgow coma scale （GCS） score, survival rate and incidence of complication were compared between the two groups before and after surgery. Results The GCS score of observation group was significantly higher than that of control group. The median shift, survival rate and incidence of complication were significantly lower in observation group than those in control group, the difference was significant （P 〈 0. 05）. Conclusions The standard big-bone craniectomy for hematoma evacuation has significant clinical effects in the treatment of severe craniocerebral injury in lateral fissure area, which could effectively improve the GCS score and long-term survival rate with fewer complications, and it ishelpful to improve the quality of life and worthy of clinical application.
