摘要
目的观察双侧标准外伤大骨瓣开颅手术治疗重型颅脑损伤的临床效果。方法选取2018年1月—2021年10月重型颅脑损伤患者50例为研究对象,根据动态随机化分组,分为对照组(n=25,大骨瓣开颅手术治疗)和研究组(n=25,双侧标准外伤大骨瓣开颅手术治疗)。对比两组患者颅内压及脑灌注压;血流动力学指标;脑氧代谢水平;并发症发生率。结果①与对照组比较,研究组颅内压(16.61±1.23)mmHg、脑灌注压(62.38±1.31)mmHg更低,差异有统计学意义(t=17.585、17.118,P<0.05)。②研究组血流动力学指标优于对照组,差异有统计学意义(P<0.05)。③研究组比对照组患者脑氧代谢水平改善明显,差异有统计学意义(P<0.05)。④研究组并发症发生率(4.00%)显著低于对照组(32.00%),差异有统计学意义(χ^(2)=4.878,P=0.027)。结论双侧标准外伤大骨瓣开颅手术治疗重型颅脑损伤疾病的临床疗效显著,可改善患者颅内压、脑灌注压等指标,临床治疗安全可靠,有临床推广意义。
Objective To observe the clinical effect of bilateral standard trauma craniotomy for severe craniocerebral injury.Meth⁃ods 50 patients with severe craniocerebral injury from January 2018 to October 2021 were selected as the research objects.Ac⁃cording to dynamic randomization,they were divided into the control group(n=25,large bone flap craniotomy)and the study group(n=25,bilateral standard trauma craniotomy with large bone flap).The intracranial pressure and cerebral perfusion pres⁃sure;hemodynamic indexes;cerebral oxygen metabolism level;incidence of complications between the two groups were com⁃pared.Results 1.Compared with the control group,the intracranial pressure(16.61±1.23)mmHg and cerebral perfusion pres⁃sure(62.38±1.31)mmHg in the study group were lower,and the differences were statistically significant(t=17.585,17.118,P<0.05).2.The hemodynamic indexes of the study group were better than those of the control group,and the difference was statis⁃tically significant(P<0.05).3.Compared with the control group,the cerebral oxygen metabolism level of the study group was significantly improved,and the difference was statistically significant(P<0.05).4.The incidence of complications in the study group(4.00%)was significantly lower than that in the control group(32.00%),and the difference was statistically significant(χ^(2)=4.878,P=0.027).Conclusion Bilateral standard trauma craniotomy in the treatment of severe craniocerebral injury has a signifi⁃cant clinical effect,and can improve the patients'intracranial pressure,cerebral perfusion pressure and other indicators.The clinical treatment is safe and reliable,and has clinical significance.
