摘要
目的 探讨血清尿酸水平对癫痫与晕厥的鉴别诊断价值.方法 选取2012-11~2017-04在北京积水潭医院神经内科急诊就诊并诊断为癫痫或晕厥的患者,其中癫痫患者106例,晕厥患者102例,发病12 h以内抽静脉血,用尿酸酶法测定血清尿酸水平.结果 癫痫组患者的血清尿酸水平显著高于晕厥组[(529.28±202.35) μmol/L vs.(306.84±76.02)μmol/L,t=10.417,P<0.01].癫痫组患者血清尿酸水平升高的比例显著高于晕厥组(71.70% vs.8.82%,x2=85.035,P<0.01).癫痫组患者血清尿酸水平与年龄呈负相关(r=-0.302,P=0.002);晕厥组患者血清尿酸水平与年龄无相关性(P=0.653).对25例尿酸升高的癫痫患者进行随访,22例(88%)患者在癫痫发作后3d内血清尿酸水平降至正常,其余3例(12%)血清尿酸水平较前也有显著下降.结论 血清尿酸水平对癫痫与晕厥的鉴别有一定的临床指导价值.
Objective To investigate the value of serum uric acid level in the differential diagnosis of epilepsy and syncope.Methods A total of 208 patients with epilepsy (106 patients) and syncope (102 patients) who visited the neurology emergency department of Beijing Jishuitan Hospital from November 2012 to April 2017 were enrolled into this study.Venous blood was collected within 12 hours of onset to measure the serum uric acid level by uricolase method.Results The serum uric acid level of the epilepsy group (529.28 ± 202.35) μmol/L was significantly higher than that of the syncope group (306.84 ±76.02) μmol/L(t =10.417,P <0.01).The proportion of elevated serum uric acid level in epilepsy group (71.70%) was significantly higher than that (8.82%) in syncope group (x2 =85.035,P <0.01).Serum uric acid levels were negatively correlated with age in the epilepsy group (r =-0.302,P =0.002).Serum uric acid levels were not associated with age in the syncope group (P =0.653).25 cases of epilepsy with elevated uric acid were followed up:serum uric acid levels dropped to normal within 3 days after the onset of epilepsy in 88% of the patients (22 patients),serum uric acid levels of the other 3 cases (12%) were significantly lower than before.Conclusion Serum uric acid level has certain clinical value for the differentiation of epilepsy and syncope.
