摘要
目的分析脑卒中患者合并下肢静脉血栓(deep vein thrombosis,DVT)的危险因素。方法选择2014年1月—2016年1月在我院神经内科住院的脑卒中合并下肢静脉血栓患者30例作为观察组。选取同期住院患者中脑卒未合并下肢静脉血栓患者40例作为对照组。对两组相关资料进行分析。结果脑卒中合并DVT患者卧床时间更长、身体质量指数(BMI)高、脱水药物应用时间长,下肢肌力更差,多合并肺感染、糖尿病或高血压。而低蛋白血症及贫血在两组间对比,差异不具有统计学意义(P>0.05)。结论下肢肌力<Ⅲ级、BMI≥25 kg/m^2、卧床时间≥7天、应用脱水药≥7天、合并肺感染、高血压、糖尿病等是脑卒中患者发生DVT的危险因素,对存在危险因素患者应加强预防,避免DVT的发生。
Objective To investigate the risk factors for deep venous thrombosis（DVT） of the lower extremity in stroke patients. Methods 30 patients with stroke and lower limb venous thrombosis who were hospitalized in neurology department of our hospital from January 2014 to January 2016 were selected as the observation group. A total of 40 patients with mid-cerebral stroke complicated with venous thrombosis of lower extremities were selected as control group. The related data of the two groups were analyzed. Results Patients in the experimental group had longer time in bed, high BMI, long duration of dehydration medication, lower muscle strength in the lower limbs, and more complicated pulmonary infection, diabetes or hypertension. However, the difference of hypoproteinemia and anemia between the two groups was not statistically significant（P0.05）. Conclusion Lower limb muscle strength ≤ Ⅲ grades, BMI ≥ 25 kg/m^2, bed time ≥ 7 days, Dehydration drug application ≥ 7 days, complicated pulmonary infection, diabetes or hypertension may be the risk factor of DVT in stroke patients. Patients with risk factors should be strengthened to prevent DVT.
