摘要
目的探讨彩色多普勒超声在甲状腺良恶性结节鉴别诊断中的应用价值。方法选取2014年1月—2016年12月我院经手术证实的甲状腺结节患者80例,分析结节的超声声像图表现及血流分布情况,结节血流分布情况采用Adler分级评价标准。结果 80例甲状腺结节患者中,经病理检查确诊良性57例,恶性23例。良性结节包括甲状腺腺瘤18例,结节性甲状腺肿33例,亚急性甲状腺炎4例,桥本氏甲状腺炎2例;恶性结节包括甲状腺乳头状癌14例、甲状腺滤泡癌5例、甲状腺未分化癌1例和甲状腺髓样癌3例。恶性结节的血流较良性结节明显丰富,良性结节以I、II型血流为主,恶性结节以Ⅲ型血流分布为主;恶性结节内血流平均收缩期峰值血流速度(PSV)、阻力指数(RI)明显高于良性结节(P<0.05),多呈现高阻频谱。结论甲状腺结节在良性与恶性上有其自身的声像图及血流特点,彩超是诊断甲状腺结节的首先检查方法,在临床上值得推广和应用。
Objective: To evaluate the differential diagnosis value of color Doppler ultrasound in benign and malignant thyroid nodules. Methods: Eighty cases of thyroid nodule were selected in our hospital from January 2014 to December 2014,the sonographic findings within the nodules and the surrounding blood flow distribution were observed and rich,and Adler grading evaluation criterion was used for nodules blood flow. Results: of 80 cases of patients with thyroid nodules,57 benign cases and 23 malignant cases were confirmed by pathological examination. The benign cases included 33 cases of nodular goiter,18 cases of thyroid adenoma,4 cases of subacute thyroiditi,in 2 cases of hashimoto’s thyroiditis,and malignant cases included 14 cases of papillary thyroid carcinoma,5 cases of thyroid follicular carcinoma,3 cases of medullary thyroid carcinoma and 1 case of thyroid anaplastic carcinoma. There was more obvious rich blood flow in the malignant nodules than in benign nodules,blood flow distribution was Ⅲ model; Malignant nodules flow average PSV and RI were higher than benign nodules( P 〈 0. 05),and had more high impedance spectrum. Conclusion: Benign and malignant thyroid nodules have its own sonographic characteristics,and color Doppler ultrasound has certain clinical value for the identification of benign and malignant thyroid nodules,which is worthy of clinical popularization and application.
