摘要
目的观察三平面截骨在外翻矫形中的应用。本文介绍一种改良的Chevron截骨术在外翻畸形中的疗效。方法选取我院25例(39足)中度外翻患者,以改良Chevron三平面截骨术进行治疗。对手术前后患者的外翻角(HVA)、第1跖骨间角(IMA)、第1跖骨远端关节面角(DMAA)进行测量对比及AOFAS足部功能进行观察。结果随访1年,术后25例患者截骨端全部愈合,平均愈合时间为6~8周,未发生并发症。术后患者HVA、IMA、DMAA显著降低,跖痛症状消失。AOFAS评分显著增加,差异有统计学意义(P<0.05)。结论改良的三平面Chevron截骨术可有效对中度复合外翻进行矫形治疗。
Objective Chevron osteotomy is widely used in treating symptoms of mild to moderate hallux valgus deformity. In this artical, describes the clinical effect of a modified Chevron osteotomy in the composite hallux valgus deformity. Methods A total of 25 cases in our hospital (39 feet) with moderate composite hallux valgus. All patients has moderate hallux valgus deformity associated with DMAA angle increases, the second metatarsal bone metastasis of metatarsalgia, were treated by modified Chevron triplanar osteotomy. All patients with hallux valgus angle (HVA), the first metatarsal angle (IMA), the first metatarsal distal articular surface angle (DMAA) was measured and AOFAS foot function were observed in before and after operation. Results All the patients were followed up for one years. All the 25 patients were healed with the average healing time of 6-8 weeks. No complications occurred. Postoperative HVA,IMA,DMAA were significantly reduced, and the symptoms disappeared. AOFAS score increased significantly, the difference was statistically significant (P〈0.05). Conclusion Modified Chevron triplanar osteotomy can effectively improved to moderate composite correction of hallux valgus.
