摘要
目的探讨TP化疗方案（奈达铂＋紫杉醇）与三维适形放疗联合治疗中晚期食管癌的疗效。方法选取中晚期食管癌患者83例，依照治疗方式分组，对照组41例予以三维适形放疗，研究组42例在对照组基础上联合TP化疗方案治疗。比较两组疗效、生存质量（SF-36）评分、不良反应发生率，比较治疗前后两组血清细胞角蛋白19片段抗原21-1（CYFRA21-1）、糖类抗原125（CA125）、癌胚抗原（CEA）水平。结果研究组总有效率（69．05％）高于对照组（46．34％），差异有统计学意义（P〈0．05）；研究组治疗后血清CYFRA21—1、CA125、CEA水平低于对照组，差异有统计学意义（P〈0．05）；治疗后两组SF-36评分较治疗前显著升高，且研究组SF-36评分高于对照组，差异有统计学意义（P〈0．05）；研究组不良反应总发生率（11．90％）与对照组（4．88％）比较差异未见统计学意义（P〉0．05）。结论中晚期食管癌患者予以TP化疗方案与三维适形放疗联合治疗效果显著，可显著降低其血清肿瘤标志物水平，提高其生存质量，且安全性较高。
Objective To investigate the efficacy of TP chemotherapy （nedaplatin ＋ paclitaxel） combined with three-dimensional conformal radiotherapy on patients with mid-stage and advanced esopha- geal cancer. Methods A total of 83 patients with advanced esophageal cancer were selected and divided into two groups according to the different treatment method, 41 cases in the control group were treated by three dimensional conformal radiotherapy, and 42 cases in the study group were treated by TP chemother- apy based on the control group. The efficacy, quality of life （SF-36） , and the incidence of adverse reac- tions were compared between the two groups. The levels of serum cytokeratin 19 fragment antigen 21-1 （CYFRA21-1）, carbohydrate antigen 125 （CA125） and carcinoembryonic antigen （CEA） in the two groups before and after therapy were compared. Results The total effective rate of the study group （69. 05% ） was higher than that of the control group （46. 34% ）, the difference was significant （ P 〈 0. 05 ）. The levels of serum CYFRA21-1, CA125 and CEA in the study group were lower than those of the control group, and the differences were significant （P 〈 0. 05）. After treatment, the scores of SF- 36 in the two groups were significantly higher than that before the treatment, and the SF-36 score in the study group was higher than that in the control group, and the differences were significant （ P 〈 0. 05 ）. There was no significant difference in the total adverse reaction rate between the study group （11.90% ） and the control group （4. 88%, P 〉 0. 05 ）. Conclusions TP chemotherapy combined with three di- mensional conformal radiotherapy is effective on advanced esophageal cancer, which can significantly re- duce the levels of serum tumor markers and improve their quality of life and the safety is high.
