摘要
目的研究西南医科大学附属医院临床分离的大肠埃希菌耐多药与产超广谱β-内酰胺酶(产ESBLs)的关系,为其治疗提供理论依据。方法采集西南医科大学附属医院2014年7月-2014年11月临床分离的155株大肠埃希菌,采用纸片扩散法进行药敏分析,并以双纸片法确定产ESBLs。结果 155株大肠埃希菌耐药情况依次为AMP 89.68%、AMC 88.39%、SXT 75.48%、CTX 58.06%、GEN 49.03%、CIP 47.10%、LEV33.55%、CAZ 28.39%、ATM 19.35%、FEP 10.97%、FOX 8.39%、AMK 2.58%、CFS 1.29%、IMP 0.00%。产ESBLs率为62.58%,多耐占57.42%(89),双耐占22.58%(35),单耐占12.90%(20),全敏占7.10%(11)。产ESBL菌株对AMP、AMC、CAZ、CTX、FEP、ATM、LEV、CIP、GEN、SXT耐药率高于非产ESBLs菌株,多耐菌株产ESBLs率(78.61%)高于单耐菌株(25.00%)。结论本地区大肠埃希菌耐多药情况突出,多耐菌株产ESBLs率高于单耐菌株。医院感染菌株中产ESBLs及耐药菌株比例高,临床需重点监控。
Objective To explore the relationship between multi-drug resistance and extended-spectrum β-lactamases(ESBLs) in Escherichia Coli(E.Coil).Methods 155 isolated E.Coil strains were collected; disk diffusion was applied in testing the drug sensitivity while double-disk diffusion was applied in determining ESBLs.Results The drug resistance of the 155 strains ranged as follows:AMP(89.68%),AMC(88.39%),SXT(75.48%),CTX(58.06%),GEN(49.03%),CIP(47.10%),LEV(33.55%),CAZ(28.39%),ATM(19.35%),FEP(10.97%),FOX(8.39%),AMK(2.58%),CFS(1.29%) and IMP(0%); the rate of ESBLs strains was62.58%; multi-resistance took up 57.42%,double-resistance 22.58%,single-resistance 13.33% and total sensitivity 7.10%; the drug-resistance of ESBLs strains to AMP,AMC,CAZ,CTX,FEP,ATM,LEV,CIP,GEN and SXT was higher than that of ESBLs negative strains,and the ESBLs rate of multi-resistant strains(78.61%) was higher than that of single-resistant strains(25.00%).Conclusions Multi-drug Resistance of E.coli is prevalent; the ESBLs rate in multi-drug resistant strains is higher than that in single-resistant strains; ESBLs and drug-resistant strains take up a large proportion in hospital infection strains,which needs key monitoring in clinical.
