摘要
目的观察视网膜分支静脉阻塞黄斑水肿患者玻璃体腔内注射康柏西普和曲安奈德后的治疗效果,为康柏西普和曲安奈德选择用药提供临床依据。方法选取2016年6月—2017年6月我院接收的36例(36眼)视网膜颞上分支静脉阻塞黄斑水肿患者,以玻璃体腔注射康柏西普和曲安奈德分为二组,观察药物注射后7d,28d患者眼部变化情况,包括视力,眼压,裂隙灯显微镜,视网膜光学相干断层扫描检查(OCT)。第30天、60天继续第2次,第3次注射相同药物,观察相同指标变化共4个月。结果玻璃体腔注射康柏西普组在7d时黄斑水肿基本消退,而曲安奈德组28d时消退,28d后未见复发。第2次,第3次注射相同药物剂量维持不变,视力变化不明显。但曲安奈德组眼压升高,平均增长(4.21+1.22)mmHg。曲安奈德组加重晶体后囊下浑浊,康柏西普组晶体混浊无变化。结论玻璃体腔注射康柏西普和曲安奈德对视网膜分支静脉阻塞黄斑水肿具有明显效果,但曲安奈德容易引起眼压升高和加重白内障。
Objective To observe clinical effect of the branch vein occlusion in patients with macular edema that treated with Conbercept or Triamcinolone acetonide,and to provide clinical basis for the application of Conbercept and Triamcinolone acetonide.Methods A retrospective analysis was performed in June 2016-June 2017 from the temporal branch retinal vein occlusion36 cases（36 eyes）with macular edema patients.All patients were divided into two groups depend on Conbercept and Triamcinolone acetonide,and then the ocular changes were observed on drug injection after 7 days,28 days,including visual acuity,intraocular pressure,slit lamp microscope,optical coherence tomography（OCT）.Same indicators were also observed after 30 days,60 days with injection of the same drug up to the fourth month.Results After intravitreal,macular edema in Conbercept group was disappear on 7 days,but it receded 28 days in Triamcinolone acetonide group.Patients of two groups did not see a relapse after 28 days.After the second and third injection of the same doses remain unchanged,vision change was not obvious.In Triamcinolone acetonide group,patients＇ iop elevated4.21 mmHg on average,and whose lens posterior capsule was aggravating,but no changed in Triamcinolone acetonide group.Conclusion The effect of treating with Conbercept and Triamcinolone acetonide for macular edema with branch retinal vein occlusion is obvious,but Triamcinolone acetonide is easy to cause elevated iop and subcapsular cataract.
