行动导向宣教对低位直肠癌行结肠造瘘患者自护能力、自我效能的影响研究 被引量:12

摘要 目的研究行动导向宣教对结肠造瘘患者自护能力和自我效能的影响。方法纳入60例结肠造瘘患者作为研究对象,随机分为两组,每组各30例。观察组采用行动导向法进行健康宣教,对照组行常规健康教育。比较两组护理前后自护能力和自我效能变化情况,分析自护能力与自我效能的关系。结果两组护理后护理技能、健康知识及自护能力总分均高于护理前(均P<0.05),观察组护理后护理技能、自我概念、自护责任感、健康知识水平及自护能力总分均高于对照组(均P<0.05)。两组护理后积极态度、减轻压力、自我决策及自我效能总分评分均高于护理前(均P<0.05),观察组护理后各项评分均高于对照组(均P<0.05)。两组患者护理后自我效能总分和自我护理能力总分呈线性相关(r=0.9389,P<0.001)。经Pearson偏相关分析发现,自我护理能力评分与自我效能各维度评分均不同程度的正相关(均P<0.05)。结论行动导向宣教能显著提高结肠造瘘患者术后自我护理能力,改善自我效能,结肠造瘘患者自我效能和自我护理能力可能存在相互促进作用。 Objective To investigate the effect of action-oriented education on self-care ability and self-efficacy in patients with low rectal cancer undergoing colostomy. Methods 60 patients with colostomy were randomly assigned to two groups, with 30 cases in each group. The treatment group received action-oriented education and the control group received routine health education. The changes of self-care ability and self-efficacy before and after nursing were compared between the two groups. The relationship between self-efficacy and self-care ability were analyzed. Results After nursing, the scores of nursing care skills, health knowledge and self-care ability were higher than before nursing in both groups(all P 〈 0.05). The scores of nursing care skills, self-concept, selfresponsibility, health knowledge level, and self-care ability were higher in the treatment group than in the control group(all P 〈 0.05).The scores of positive attitude, relief pressure, self-decision and self-efficacy were higher than before nursing in both groups(all P 0.05). The scores were higher in the treatment group than in the control group(all P 〈 0.05). There was a linear correlation between the total score of self-efficacy and the total score of self-care ability after nursing in the two groups(r = 0.9389, P 〈 0.001). Pearson partial correlation analysis showed that the score of self-nursing ability and the scores of self-efficacy were positively related across different scales(all P 〈 0.05). Conclusion The action-oriented education can significantly improve the self-care ability and self-efficacy of patients with colostomy. The self-efficacy and self-nursing ability of patients with colostomy may promote each other.
