摘要
目的了解南京市社区2型糖尿病并发高血压患者的糖化血红蛋白控制情况及其影响因素,为制定相应防制措施提供依据。方法选取纳入南京市化工园区5个社区卫生服务中心管理的1 399例2型糖尿病患者开展问卷调查、体格检查和实验室检查,比较糖尿病合并高血压组和血压正常组的临床指标,采用logistic回归分析影响糖化血红蛋白控制的因素。结果 2型糖尿病合并高血压组的体质指数、总胆固醇平均值、并发症发生率均高于血压正常组,差异有统计学意义(P均<0.05)。多因素logistic回归分析显示,教育程度高(>12年)、甘油三酯达标及低密度脂蛋白胆固醇达标的合并高血压患者糖化血红蛋白控制效果较好,OR(95%CI)分别为1.71(1.01,2.91)和1.57(1.13,2.17)、1.69(1.14,2.50);间断服药、血压水平偏高及病程长(≥10年)合并高血压患者糖化血红蛋白控制效果差,OR(95%CI)分别为0.33(0.14,0.79)、0.63(0.41,0.95)和0.61(0.44,0.83)。结论应采取综合性防制措施对糖尿病合并高血压患者进行管理,从而更有效降低糖尿病并发症的发生率,提高患者生活质量。
[Objective] To understand the control status of glycosylated hemoglobin and its influencing factors among type2 diabetes patients with hypertension in community of Nanjing City,provide the basis for the formulation of prevention and control measures.[Methods]1 399 patients with type 2 diabetes were recruited from 5 community health service centers in Huagongyuan District of Nanjing City,and received questionnaire survey,physical examination and laboratory examination. The clinical indexes were compared between the diabetes combined with hypertension group and the normal blood pressure group. The logistic regression was used to analyze the factors influencing the control of glycosylated hemoglobin.[Results]The body mass index,total cholesterol level and incidence of complications in type 2 diabetes combined with hypertension group were higher than those of the normal blood pressure group,and the differences were statistically significant(all P0.05). Multiple-factor logistic regression showed that higher education level(12 years),lower triglycerides,lower low-density lipoprotein cholesterol were protective factors for control of glycosylated hemoglobin in patients with type 2 diabetes p atients and hypertension,OR(95% CI) was respectively 1.71(1.01,2.91),1.57(1.13,2.17) and 1.69(1.14,2.50),whereas intermittent medication,higher blood pressure,long course of diabetes(≥10 years) were risk factors,and OR(95% CI) was respectively 0.33(0.14,0.79),0.63(0.41,0.95) and0.61(0.44,0.83).[Conclusion]Comprehensive prevention measures should be taken to manage diabetes mellitus patients with hypertension,so as to reduce the incidence of complications of diabetes more effectively and improve the quality of life of patients.
