68例老年轻中度肺部感染患者肠道菌群分析 被引量:5

摘要 目的分析68例老年轻中度肺部感染患者肠道菌群。方法选取邯郸市第一医院2014年1月—2016年1月收治的老年轻中度肺部感染患者68例,根据痰培养结果分为单纯球菌感染者20例、单纯杆菌感染者26例、混合感染者22例;根据治疗情况分为单纯采用抗生素治疗者37例,采用抗生素联合肠道微生态制剂治疗者31例。比较不同病原菌感染类型患者肠道菌群菌落计数,比较单纯采用抗生素治疗患者及采用抗生素联合肠道微生态制剂治疗患者治疗前后肠道菌群菌落计数。结果不同病原菌感染类型患者肠球菌、肠杆菌、酵母菌、双歧杆菌、拟杆菌及乳酸杆菌菌落计数比较,差异无统计学意义(P>0.05)。单纯采用抗生素治疗患者治疗后肠球菌、肠杆菌、酵母菌、双歧杆菌、拟杆菌及乳酸杆菌菌落计数低于治疗前(P<0.05)。采用抗生素联合微生态制剂治疗患者治疗后肠杆菌、酵母菌及乳酸杆菌菌落计数低于治疗前(P<0.05);但肠球菌、双歧杆菌及拟杆菌菌落计数与治疗前比较,差异无统计学意义(P>0.05)。结论病原菌种类与老年轻中度肺部感染患者肠道菌群改变无明显关系;肠道微生态制剂有助于预防老年轻中度肺部感染患者肠道菌群失调。 Objective To analyze the intestinal flora in 68 elderly patients with mild to moderate pulmonary infection. Methods A total of 68 elderly patients with mild to moderate pulmonary infection were selected in the First Hospital of Handan from January 2014 to January 2016, and they were divided into A1 group(with coccal infection only, n=20), A2 group(with bacillus infection only, n=26) and A3 group(with mixed infection, n=22) according to sputum culture results, into B1 group(treated by antibiotics only, n=37) and B2 group(treated by antibiotics combined with intestinal microecological preparation, n=31) according to therapeutic methods. Colony counting of intestinal flora was compared in A1 group, A2 group and A3 group, in B1 group and B2 group before and after treatment. Results No statistically significant differences of colony counting of enterococcus, enteric bacilli, saccharomycetes, bifidobacterium, bacteroides or lactobacillus was found in A1 group, A2 group and A3 group(P〉0.05). After treatment, colony counting of enterococcus, enteric bacilli, saccharomycetes, bifidobacterium, bacteroides and lactobacillus in B1 group was statistically significantly lower than that before treatment, respectively(P〈0.05). After treatment, colony counting of enteric bacilli, saccharomycetes and lactobacillus in B2 group was statistically significantly lower than that before treatment, respectively(P〈0.05), while no statistically significant differences of colony counting of enterococcus, bifidobacterium or bacteroides was found compared with that before treatment, respectively(P〉0.05). Conclusion Pathologic bacteria is not significantly correlated with intestinal flora change in elderly patients with mild to moderate pulmonary infection; intestinal microecological preparation is helpful to guard against alteration of intestinal flora in elderly patients with mild to moderate pulmonary infection.
