摘要
目的:应用锥形束CT(CBCT)观察下颌第二前磨牙根管系统解剖形态的增龄性变化,为临床治疗提供一定的参考。方法:依照纳入排除标准,筛选自2015年11月-2017年11月于解放军总医院口腔科就诊的患者,并搜集328颗下颌第二前磨牙,按照年龄依次分为6组:21-30岁,31-40岁,41-50岁,51-60岁,61-70岁,>70岁。分别获取六组年龄人群的口腔颌面部锥形束CT影像资料。观察并统计如下指标:根管类型比例(Vertucci分类法),以及颊侧髓角与颊侧釉牙骨质界交角和颊舌侧釉牙骨质界(ECJ)连线的交角(BB-BL)。六组之间采用卡方检验进行两两比较,并采用Graph Pad Prism进行柱状图(平均值及标准差)分析。结果:各年龄组根管类型分布结果显示,I型根管类型最为多见,比例高达96.6%(317/328),组间比较差异无统计学差异(P>0.05);另有约3.6%(11/328)的牙齿出现V型根管,年龄组比较无统计学差异(P>0.05)。颊髓角(Buccal pulp angle)与颊侧釉牙骨质界连线(Buccal enamelo-cemental junction)和颊舌侧釉牙骨质界(Buccal-Lingual enamelocemental junction)连线的交角(BB-BL)显示,50岁之前该牙齿交角平均在36.1°左右,且无明显增龄性改变,进入50岁以后的交角降低为28.9°,而70岁以后的交角则明显变小为23.9°,且差异具有统计学意义(P<0.05)。结论:CBCT的应用使得根管系统研究变得简单而精确,增龄性改变对下颌第二前磨牙根管类型无明显影响,但该因素引起根管的钙化将使得根管髓腔缩窄,颊侧髓角降低,BB-BL角降低,从而增加了根管系统的复杂性。
Objective:Observing the age-related changes of root canal system of mandibular second premolars by using cone beam CT(CBCT),so as to provide some references for clinical treatment.Methods:According to inclusion and exclusion criteria,a total of 328 mandibular second premolars were selected from the Department of Stomatology of PLA General Hospital in November 2015-2017 November.They were divided into 6 groups in the light of their age:21-30,31-40,41-50,51-60,61-70 years old,70 years old,and obtain the CBCT images of the six groups.Observe and statistics the following indicators:root canal type proportion(Vertucci theory),and connection angle of buccal pulp horns and buccallingual enamel junction(ECJ).The six groups were compared with the chi square test,and the GraphPad Prism was used to analyze the histogram(average and standard deviation).Results:The results showed that type I root canal type was the most common,the proportion is as high as 96.6%(317/328),which accounts for the highest proportion in all age groups,and the difference was not statistically significant(P〈0.05);and about 3.6% teeth(11/328)type V root canal were found,and there was no significant difference in age group(P〈0.05).The connection angle of BB-BL displayed,before the age of 50,the average angle was about 36.1 degrees,and there is no obvious age-related changes,but the angle reduced to 28.9 degrees after 50 years of age,while the angle is significantly smaller for 23.9 degrees after the age of 70,and the difference was statistically significant(P〈0.05).Conclusion:The application of CBCT makes the study of the root canal system become simple and accurate,the age has no effects on the root canal type,but the calcification of root canal because of aging factors will make the root medullary cavity narrow,BB-BL angle decreased,which increases the complexity of root canal system.
