摘要
目的分析儿童呼吸道肺炎链球菌的感染状况、流行趋势、血清学分型特点及耐药现状,为预防和控制肺炎链球菌感染提供依据。方法回顾性分析2013年1月-2016年12月呼吸道肺炎链球菌感染患儿的临床资料,采用荚膜肿胀试验进行血清学分型,纸片扩散法测定肺炎链球菌对13种抗菌药物的敏感性。结果 4年间共分离出肺炎链球菌394株,检出率为12.1%,多发于0岁~3岁患儿;血清学分型以19F、9A为主,分别占64.2%、15.5%;肺炎链球菌对常用抗菌药物的耐药率总体呈上升趋势,对阿奇霉素、克林霉素、红霉素、复方新诺明和四环素的耐药率均高于85.0%,多重耐药率达73.4%,未发现耐万古霉素、替考拉宁和利奈唑胺的菌株。结论肺炎链球菌在冬春季节高发,易引起3岁以下患儿呼吸道感染,肺炎链球菌对不同抗菌药物的耐药性随着时间而变化,临床应根据药敏试验结果合理选用抗菌药物。
Objective To analyze the infection status,epidemic trend,serological characteristics and drug resistance status of S.pneumoniae in children with respiratory tract,so as to provide basis for the prevention and control of S.pneumoniae infection.Methods The clinical data of children with respiratory tract S.pneumoniae infection from January 2013 to December 2016 were retrospectively analyzed,serological tests were carried out using capsule swelling test,and the sensitivity of S.pneumoniae to 13 antimicrobial agents was determined by paper diffusion method.Results A total of 394 strains of S.pneumoniae were isolated in the past four years,and the detection rate was 12.1%,mainly in 0-3 years old children;serotype was mainly composed of19 F and 9 A,accounting for 64.2% and 15.5%,respectively;the resistance rate of S.pneumoniae to the commonly used antimicrobial was on the rise,and the resistance rates of it to azithromycin,clindamycin,erythromycin,cotrimoxazole and tetracycline were higher than 85.0%,with the multiple drug resistance rate of 73.4%,and no strains resistant to vancomycin,teicoplanin and linezolid were found.Conclusion The incidence of S.pneumoniae was high in winter and spring,easy to cause respiratory tract infection in children under 3 years old,the resistance of S.pneumoniae to different antimicrobial agents varies over time,so rational selection of antibiotics should be based on drug susceptibility test results.
