摘要腰椎间盘突出症是骨科临床常见病和多发病,非手术疗法以往一直被视为首选治疗方法。但随着脊柱微创手术技术的发展和推广,腰椎间盘突出症首选非手术治疗的原则受到了前所未有的挑战。面对腰椎间盘突出症,是否应首选非手术疗法?如何预测突出物的重吸收?如何处理巨大(或游离)型腰椎间盘突出症?微创介入技术、显微内窥镜技术、经皮椎间孔镜技术存在哪些问题?本文对腰椎间盘突出症非手术治疗和微创手术治疗中存在的这些问题进行了探讨。
