摘要
目的该课题旨在探讨子宫肌瘤患者应用B型超声诊断的临床价值。方法将该院2015年2月—2017年6月在院治疗的72例子宫肌瘤患者作为研究对象,对其应用GELOGIQ-3型超声诊断仪行阴道和腹部B超检查,根据术后病理检查结果分析B型超声的诊断效果。结果 72例子宫肌瘤患者的B型超声确诊率为88.89%,确诊的黏膜下肌瘤、浆膜下肌瘤、肌壁间肌瘤、多发性肌瘤分别为28例、14例、10例、12例,5例子宫腺肌病、1例卵巢纤维瘤和2例宫颈肥大共8例患者误诊。B型超声影响特征为低回声型的25例患者子宫腔内回声光点的亮度偏低,外凸,病灶回声低,呈边界清晰的圆形或类圆形病变;高回声型的18例患者病灶回声强,子宫腔高回声结节状光点,在子宫部位边界清晰的团块周围是黯淡光环;混合回声型的14例患者子宫肌瘤生长缺血引起病灶脂肪样、水肿、钙化、坏死,肌瘤病灶边界清晰,不均匀强弱回声、不规则回声、无回声区可同时存在;中等回声型的15例患者内部是中等回声结节状光点,边界清晰的子宫团块。结论 B型超声在子宫肌瘤诊断中显像清晰,影响特征明显,诊断确诊率较高,值得推广使用。
Objective To study the clinical value of type B ultrasound in diagnosis of hysteromyoma patients. Methods72 cases of patients with hysteromyoma treated in our hospital from February 2015 to June 2017 were selected as the research objects, the vaginal and abdominal B-ultrasound examination was conducted by the GELOGIQ-3 type ultrasound instrument, and the diagnosis results of type B ultrasound was analyzed by the postoperative pathological examination results. Results The confirmation rate of type B ultrasound of 72 cases of hysteromyoma patients was 88.89%,and the submucous myoma, subserous myoma, myoma of myomicula, multiple myoma were respectively in 28 cases, 14 cases, 10 cases and 12 cases, and there were 8 misdiagnosed cases, including 5 cases with adenomyosis, 1 case with fibroma ovarii and 2 cases with cervical hypertrophy, the brightness of uterine cavity echo light point of 25 cases whose type B ultrasound feature was low echo type, showing evagination, circular or roundlike lesions with clear boundary,and the lesion echo of 18 cases with high echo was strong, and 14 cases of patients with mixed echo type had the lesion fat-like, edema, calcification and necrosis caused by the hysteromyoma growth ischemia, showing the clear lesion boundary, uneven weak and strong echoes, irregular echo, and echoless area, and 15 cases of patients in the middle echo type had the middle echo nodule light point, with uterus masses of clear boundary. Conclusion The image of type B ultrasound in diagnosis of hysteromyoma is clear, with obvious imaging features and high diagnosis confirmation rate,which is worth promotion and application.
