摘要
目的探讨联合经阴道超声检查与单纯经腹部超声检查对子宫腺肌病的诊断价值。方法选取2009年6月至2013年6月间在汕头大学医学院第一附属医院妇科收治的术后病理证实为子宫腺肌病患者228例作为研究对象,进行回顾性分析,比较联合经阴道超声检查与单纯经腹部超声检查对子宫腺肌病的诊断符合率及诊断价值,同时分析两种检查的诊断特点。结果①联合经阴道超声检查组的诊断符合率明显高于单纯经腹部超声检查组(P<0.05);②ROC分析显示,两组均对子宫腺肌病有显著的诊断价值(P=0.0290,P=0.0411),联合经阴道超声检查组的诊断价值(曲线下面积0.6551)显著高于单纯经腹部超声检查组(曲线下面积0.6351);③比较超声准确诊断为子宫腺肌病两组病例的动脉搏动指数和动脉阻力指数,未见统计学差异(P>0.05)。结论联合经阴道超声检查及单纯经腹部超声检查对于子宫腺肌病均有较高诊断价值,同时联合经阴道超声检查的诊断符合率较经腹部超声检查更优。
Objective To discuss the diagnosis value of adenomyosis in combined transvaginal ultrasound and simply by abdominal ultrasound examination.Methods Selection in June 2009 to June 2013 in the first affiliated hospital of shantou university medical college department of gynaecology treated postoperative pathology confirmed for 228 pa-tients with adenomyosis as the research object,were analyzed retrospectively,and compared to combined the transvagi-nal ultrasound with simply by abdominal ultrasound examination in the diagnosis of adenomyosis coincidence rate and the diagnostic value,at the same time,analyzing the characteristics of the two kinds of the diagnosis.Results ①Diag-nosis coincidence rate of combined transvaginal ultrasound inspection group was obviously higher than that of simply by abdominal ultrasound inspection group（P 〈0.05）;② ROC analysis shows that both groups value to the diagnosis of adenomyosis were significant（P =0.0290,P = 0.0290）,the diagnostic value of combined transvaginal ultrasound in-spection group （the area under the curve 0.6551）is significantly higher than simply by abdominal ultrasound inspection group （the area under the curve 0.6351）;③To compare the ultrasonic accurate diagnosis of adenomyosis two groups of cases of pulses and artery resistance index,no statistical difference （P〉0.05）.Conclusion Combined transvaginal ultrasound and simply by abdominal ultrasound examination for adenomyosis had higher diagnostic value,at the same time,combined transvaginal ultrasound diagnostic rate is better simply by abdominal ultrasound examination.
