基质金属蛋白酶-2和基质金属蛋白酶-9在妊娠期高血压疾病胎盘组织中表达的临床研究 被引量:6

Clinical study on expressions of matrix metalloproteinase-2 and matrix metalloproteinase-9 in placental tissue of patients with hypertensive disorder complicating pregnancy

摘要 目的:通过测定妊娠期高血压疾病患者胎盘组织中的基质金属蛋白酶-2和基质金属蛋白酶-9的表达情况,探讨妊娠期高血压疾病发病的病因。方法:选取在汕头大学医学院第一附属医院产科住院足月分娩92例孕妇,正常妊娠40例为对照组,妊娠期高血压疾病患者例52为研究组,其中轻度子痫前期27例,重度子痫前期25例,应用免疫组化法,检测MMP-2和MMP-9在胎盘组织中的表达。结果:①MMP-2在妊娠期高血压疾病患者胎盘组织中的阳性表达与正常妊娠组相比,差异有统计学意义(P<0.05);轻度子痫前期组,重度子痫前期组与正常妊娠3组比较,差异有统计学意义(P<0.05)。重度子痫前期组与轻度子痫前期组比较,尽管胎盘组织MMP-2阳性表达降低,两组间差异无统计学意义(P>0.05)。②MMP-9在妊娠期高血压疾病患者胎盘组织中的阳性表达与正常妊娠组相比,差异有统计学意义(P<0.05);重度子痫前期组与轻度子痫前期组比较,差异有显著性(P<0.05)。结论:MMP-2和MMP-9在妊娠期高血压疾病组患者胎盘组织中表达的降低,导致滋养细胞浸润能力下降,可能与妊娠期高血压疾病发病有关,且与病情轻重呈正相关。 Objective: To detect the expressions of matrix metalloproteinase - 2 ( MMP - 2 ) and matrix metalloproteinase - 9 ( MMP -9) in placental tissue of patients with hypertensive disorder complicating pregnancy ( HDCP), explore the causes of HDCP. Methods : A total of 92 full - term pregnant women were selected from obstetrical department of the hospital, and then they were divided into control group (40 normal pregnant women) and study group (52 pregnant women with HDCP) ; the women in study group were divided into mild preeclampsia group (27 women) and severe preeclampsia group (25 women) ; immunohistochemical method was used to detect the expression levels of MMP -2 and MMP -9 in placental tissue. Results: There was statistically significant difference in positive expression rate of MMP - 2 in placental tissue between study group and control group (P 〈 0.05 ) ; there was statistically significant difference in positive ex- pression rate of MMP - 2 in placental tissue among mild preeclampsia group, severe preeclampsia group, and control group ( P 〈 0. 05 ) ; the positive expression rate of MMP - 2 in placental tissue of severe preeclampsia group was lower than that of mild preeclampsia group, but there was no statistically significant difference between mild preeclampsia group and severe preeclampsia group (P 〉 0. 05) ; there was statistically significant difference in positive expression rate of MMP - 9 in placental tissue between study group and control group (P 〈 0. 05) ; there was statistically significant difference in positive expression rate of MMP - 9 in placental tissue between mild preeclampsia group and severe preeclampsia group (P 〈 0. 05 ) . Conclusion : The down - regulations of MMP - 2 and MMP - 9 in placental tissue of patients with HDCP in- duce the decrease of invasive ability of trophoblast, which may be correlated with pathogenesis of HDCP, and it is positively correlated with degree of the disease.
