摘要
目的:观察固本祛痰化瘀方治疗肺心病痰浊壅肺证的临床疗效。方法:选取80例肺心病(痰浊壅肺证)患者,按照随机方法将其分成两组,即观察组和对照组。对照组按照西医常规疗法进行治疗,观察组在西医常规治疗基础上予固本化痰方口服,治疗周期为4周,观察其临床疗效及对中医证候的改善情况,检测治疗前后动脉血气指标,相关炎症因子,免疫球蛋白和T细胞亚群的变化。结果:观察组临床疗效明显优于对照组(P<0.05),经治疗后中医症状评分与对照组比较有显著性差异(P<0.05),观察组与对照组比较PaCO_2水平明显低于对照组(P<0.05),PaO_2与对照组比较有显著性差异,明显高于对照组(P<0.05),经治疗后观察组血清TNF-α、IL-6水平均明显低于对照组(P<0.05),观察组免疫球蛋白Ig G、Ig A、Ig E水平均明显低于对照组(P<0.05),T细胞亚群比较,观察组与对照组存在显著性差异(P<0.05)。结论:固本祛痰方治疗肺心病疗效显著,具有一定的临床价值,可在临床进一步推广。
Objective： To observe the clinical curative effect of using Guben Quyu Huatan Recipe in the treatment of pulmonary heart disease due to phlegm-turbidity obstructing lung syndrome. Methods： 80 cases of pulmonary heart disease（ phlegm-turbidity obstructing the lung syndrome） were divided into observation group and controlled group by the randomly method. The controlled group was treated with conventional western medicine therapy,the observation group based on routine treatment of western medicine combined with Guben Quyu Huatan Recipe,the treating period was 4 weeks. To observe the clinical efficacy and improvement of TCM syndrome,arterial blood gas indexes were detected before and after treatment,inflammation related factors,the changes of immunoglobulin and T cell subsets. Results： The clinical curative effect of the observation group was significantly better than the controlled group（ P0. 05）,the Chinese medicine symptom score compared with the controlled group,there was significant difference（ P0. 05）,the level of PaCO_2 was significantly lower than the controlled group（ P 0. 05）,PaO_2 compared with the controlled group had significant difference,obviously higher than the controlled group（ P0. 05）,the observation group after treatment of serum TNF-a,IL-6 was significantly lower than the controlled group（ P0. 05）,the observation group of immunoglobulin Ig G,Ig A and Ig E were significantly lower than the controlled group（ P 0. 05）,T cell subsets,there were significant differences between the observed group and the controlled group（ P 0. 05）. Conclusion： Using Guben Quyu Huatan Recipe in the treatment of pulmonary heart disease due to phlegm-turbidity obstructing tlung syndrome has remarkable curative effect,and has certain clinical value,can be further popularized in clinic.
