摘要
目的分析氨氯地平联合缬沙坦在高血压治疗中的应用效果及安全性,为合理选择高血压治疗方案提供科学依据。方法选取2015年9月—2016年11月收治的116例高血压患者,利用随机数表法分为两组,每组58例。对照组给予缬沙坦胶囊治疗,观察组给予缬沙坦胶囊联合氨氯地平片治疗,对比两组治疗效果、血压变化情况、血管内皮功能[血浆NO、血清内皮素(ET)]、脉搏传导速度(PWV)、血管弹性功能[大动脉弹性指数(C1)、小动脉弹性指数(C2)]以及踝臂指数,记录两组不良反应发生情况。计量资料比较采用t检验,计数资料比较采用χ2检验,P<0.05为差异有统计学意义。结果观察组治疗总有效率(93.10%)高于对照组(77.59%),差异有统计学意义(P<0.05)。治疗后,两组24 h SBP、24 h DBP、ET、PWV水平均下降,且观察组较对照组低;两组NO、C1、C2、踝臂指数均升高,且观察组较对照组高,差异有统计学意义(均P<0.05);两组不良反应发生率,差异无统计学意义(P>0.05)。结论氨氯地平联合缬沙坦可有效降低血压,改善血管内皮功能及血管弹性。
Objective To analyze the effect and safety of amlodipine combined with valsartan in treatment of hypertension, providing scientific basis for the rational choice of treatment scheme for hypertension.Methods A total of 116 patients with hypertension selected from September 2015 to November 2016 were divided into two groups by random number table, 58 cases in each group. Control group was given valsartan, while observation group was treated with valsartan combined with amlodipine. The curative effect, changes of blood pressure, vascular endothelial function, pulse wave velocity（PWV）, vascular elasticity, ankle-branchial index and adverse reactions were compared between the two groups. Measurement data were compared by t test and enumeration data were compared by χ2 test. P〈0.05 was considered statistically significant.Results The total effective rate of observation group（93.10%） was higher than control group（77.59%）（P〈0.05）. After treatment, the levels of 24 h SBP, 24 h DBP, ET and PWV in two groups decreased, and levels in observation group was lower than control group（all P〈0.05）. The NO, C1, C2 and ankle-branchial index increased in two groups, and levels in observation group was higher than control group（all P〈0.05）. There was no statistical difference in the incidence of adverse reactions between the two groups（P〉0.05）.Conclusion Amlodipine combined with valsartan in treatment of hypertension can lower blood pressure, improve vascular endothelial function and vascular elasticity.
