摘要
胫骨平台骨折(fracture of tibial plateau)是一种高能量损伤引起的骨折,骨折常累及关节面以及关节内其它的附属结构,骨折的类型多样化,临床表现复杂化,如果不对其进行及时和恰当的处理,膝关节的正常功能及稳定性将会造成严重的影响.因此,胫骨平台骨折的治疗目的是恢复关节面的稳定性及附属结构的完整性。传统的切开复位内固定术具有创伤大且难以处理关节腔内的合并损伤等缺陷。随着关节镜技术的发展及临床的广泛应用,关节镜辅助下治疗胫骨平台骨折相对于其它方法治疗较为理想。它具有创伤小、出血少、手术时间短、术后恢复快以及可以在直视下观察骨折情况的优点,同时对骨折复位效果进行精确的评估。
Fracture of the tibial plateau is a common type in intra-articular fracture. In traditional open operation, the joint capsule usually needs to be cut to observe the reduction of articular surface, the injuries of meniscus and ligament, which easily leads to postoperative adhesion, pain and increased risk of infection as well as sensory disturbance. These all affect postoperative effects. With the development of arthroscopic technology and the concept of minimally invasive surgery, arthroscopy assisted reduction and internal fixation in the fracture of the tibial plateau are gradually prevalent with good clinical effects. This article makes a summary on surgical indications and contraindications, surgical advantages, postoperative clinical curative effects, disadvantages and its development in the future.
