关节镜辅助“C”型臂微创手术治疗胫骨平台骨折 被引量:4

摘要 目的:探讨在关节镜监护下辅助“C”型臂微创手术治疗胫骨平台骨折的方法和疗效。方法:1999-2003年治疗32例胫骨平台骨折,均为新鲜闭合性骨折,按Sehatzker分型Ⅰ型5例,Ⅱ型8例,Ⅲ型14例,Ⅳ型2例,V型2例,Ⅵ型1例。合并半月板损伤8例,交叉韧带损伤7例,侧副韧带损伤4例。在关节镜监护下辅助“C”型臂进行小切口复位内固定术,在镜下同时处理关节内合并损伤,观察治疗效果。结果:术后随访8个月-3年,全部患者无切口及关节感染,2例出现皮缘坏死。所有患者骨折均在3—5个月愈合,按相关标准评定疗效,总优良率87.5%。结论:关节镜监护下辅助“C”型臂下微创手术治疗胫骨平台骨折,具有损伤小、复位满意、能同时处理关节内合并伤及术后功能恢复快的优点,值得临床推广应用。 Objective:To explore methods and their theapeutie effect in treating tibial plateau fracture under arthroseopy and C-arm fluoroscope. Methods:From 1999 to 2003,32 cases of freshan closed tibial plateau fracture were treated at the hospital. According to Schatzker criteria, 5 cases were classified as type Ⅰ fracture, 8 cases as type Ⅱ , 14 as type m, 2 type Ⅳ, 2 type Ⅴ and 1 typeⅥ. Of all the cases in this study, 7 cases were omplieated by anterior erueiated ligament injury ,4 by collateral ligament injury and 8 by meniseal injury. The fracture were reduced and fixed under arthroseopy and C-arm fluoroscope. Meanwhile, intraartieular complications were treated as well. The curative effects were observed. Results:All the cases were followed up for 8 monthes to 3 years. There was no incision and joint infection in this group. Necrosis of wound edge occurred in 2 cases. The fractures healed in 3 to 5 months in all the patients. According to relational criterion, the rate of excellence and fineness was 87.5%. Conclusion:Arthroseopie technique assisted by C-arm fluoroscope in treating tibial plateau fractures had advantages of little injury, accurate reduction, management of associated intraartieular injuries and early recovery. It was worth of further clinical application.
