摘要
目的探讨三针法预防性横结肠造口对低位直肠癌前切除术患者的生活质量的影响。方法选取2015年12月—2016年8月某省级三甲医院普外科20例行低位直肠癌前切除并行预防性横结肠造口的患者设为观察组,选取20例传统法横结肠造口缝合方式的患者设为对照组;比较两组患者术后生活质量及造口并发症的发生情况。结果观察组生活质量量表中的功能量表得分(38.51±2.20)及总生活质量量表得分(7.97±1.10)均高于对照组得分(36.93±2.23,7.11±1.49),差异有统计学意义(P<0.05);观察组症状量表得分(13.94±1.68)及症状子量表得分(11.37±1.48)均低于对照得分(14.93±1.17,12.11±1.37),差异有统计学意义(P<0.05)。观察组与对照组发生皮肤黏膜分离、粪水性皮炎、造口出血、造口回缩的例数分别为5例、1例、2例、0例及13例、8例、9例、5例,两组比较差异有统计学意义(P<0.05)。结论采用三针法预防性横结肠造口术后并发症的发生率明显低于传统的缝合方式,生活质量也优于传统的缝合方式。
Objective To assess the impact of three stich transversostomy suture on the quality of life in patients with intestinal stoma. Methods Consecutive patients undergoing three stich transversostomy suture after resection of colon cancer were enrolled as experimental group during December 2015 and August 2016. Another 20 patients undergoing tra- ditional transversostomy suture were selected as control group. Postoperative quality of life and complication rate was compared between the two groups. Results The score of functional scale and total quality of life scale in experimental group was （38.51±2. 20） and （7.97±1.10）, the score of functional scale in control group was （36. 93±2.23） and （7. 11± 1.49）, respectively, the difference was statistically significant （ P 〈 0. 05 ）. The symptoms scale score and the symptom subscale score in the experimental group was （ 13.94 ± 1.68） and （ 11.37 ± 1.48）, the symptoms scale score and the symptom subscale score in the control group was （ 14. 93 ± 1.17）, （ 12. 11 ±1.37）, the difference was statisti- cally significant （P 〈 0. 05 ）. In experimental group, mucocutaneous separation occurred in 5 patients, fecal dermatitis in 1 case, and stoma bleeding in 2 cases. In contrnl group, mucocutaneous separation occurred in 13 patients, fecal der- matitis in 8 cases, stoma bleeding in 9 cases, and stoma retraction in 5 cases, the difference was significant （ all P 〈 0. 05）. Conclusion Compared to traditional transversostomy suture, three stich transversostomy suture is associated with lower complication rate and better quality of life.
