基于目标设定理论指导的健康教育在经尿道膀胱肿瘤电切术后患者应用效果

摘要 目的探讨基于目标设定理论指导的自我管理健康教育对经尿道膀胱肿瘤电切术后患者自我管理能力及生活质量的影响。方法选择南京医科大学附属常州第二人民医院2019年2月—2020年10月经尿道膀胱肿瘤电切术后患者85例为研究对象,采用随机数字表法分为试验组43例、对照组42例。对照组给予常规自我管理教育,试验组联合基于目标设定的自我管理健康教育。随访12个月,比较两组自我管理能力、复发率、并发症及生活质量等指标。结果试验组患者病情管理、日常生活管理、情绪管理、信息管理、自我管理能力总分高于对照组(t=7.537、3.377、4.678、3.616、6.847,P<0.05)。试验组复发率4.65%、并发症6.98%低于对照组11.90%、26.19%(χ^(2)=5.308、5.701,P<0.05)。试验组躯体功能、角色功能、情绪功能、认知功能、社会功能、整体生活质量评分高于对照组(t=3.203、2.652、2.702,4.304、2.631、3.056,P<0.05)。结论基于目标设定理论的自我管理教育有助于促进经尿道膀胱肿瘤电切术后患者自我管理能力的养成,能够有效预防术后并发症及复发率,改善患者生活质量。 Objective To investigate the effect of self-management health education based on goalsetting theory guidance on patients'self-management ability and quality of life after transurethral resection of bladder tumors(TURBT).Methods 85 patients after transurethral resection of bladder tumor from February 2019 to October 2020 in Changzhou Second People's Hospital Affiliated to Nanjing Medical University were selected as the research subjects and were divided into experimental(43 patients)and control groups(42 patients)using the random number table method.The control group was given conventional self-management education and the experimental group was given self-management health education that included goal-based setting.The two groups were followed up for 12 months,and measures of self-management ability,recurrence rate,complications,and quality of life were compared.Results The total scores of patients'illness management,daily life management,emotion management,information management,and self-management abilities in the experimental group were higher than those in the control group(t=7.537,3.377,4.678,3.616,and 6.847,respectively,P<0.05).And the recurrence rate of 4.65%and complications of 6.98%in the experimental group were lower than those of 11.90%and 26.19%in the control group,respectively(χ^(2)=5.308,5.701,P<0.05).Moreover,the scores of somatic function,role function,emotional function,cognitive function,social function,and overall quality of life were better in cases than in controls(t=3.203,2.652,2.702,4.304,2.631,and 3.056,respectively,P<0.05).Conclusions Self-management education based on goal-setting theory can help promote the development of self-management skills in patients after transurethral resection of bladder tumors,which can effectively prevent postoperative complications and recurrence rates and improve patients'quality of life.
