机械通气对多发肋骨骨折并ARDS患者呼吸功能的影响研究 被引量:4

摘要 目的观察机械通气对多发肋骨骨折并急性呼吸窘迫综合征(ARDS)患者呼吸功能的影响。方法 82例多发肋骨骨折并ARDS患者,随机分为观察组和对照组,每组41例。对照组患者给予常规治疗,观察组患者在对照组基础上给予机械通气治疗。观察比较两组患者治疗前后的呼吸频率、心率、血压、p H值、动脉氧分压、动脉二氧化碳分压、血氧饱和度、氧合指数等指标。结果治疗后,两组患者呼吸频率、心率、血压均低于本组治疗前,差异具有统计学意义(P<0.05);治疗后,观察组患者呼吸频率、心率、血压显著低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组患者p H、动脉氧分压、血氧饱和度、氧合指数显著高于本组治疗前,动脉二氧化碳分压显著低于本组治疗前,差异具有统计学意义(P<0.05);治疗后,观察组患者p H、动脉氧分压、血氧饱和度、氧合指数显著高于对照组,动脉二氧化碳分压显著低于对照组,差异具有统计学意义(P<0.05)。治疗第5天,两组血浆内皮素-1(ET-1)水平显著低于治疗前,差异具有统计学意义(P<0.05);治疗第5天,观察组ET-1水平为(40.09±10.36)μmol/L,显著低于对照组的(49.13±9.75)μmol/L,差异有统计学意义(P<0.05)。结论采用机械通气治疗多发肋骨骨折并ARDS患者,能够显著改善患者的呼吸困难症状,改善患者的血气指标,纠正反常呼吸,具有临床推广价值。 Objective To observe the effect of mechanical ventilation on respiratory function of patients with multiple ribs fracture and acute respiratory distress syndrome(ARDS). Methods A total of 82 patients with multiple ribs fracture and ARDS were randomly divided into observation group and control group, with 41 cases in each group. The control group received conventional therapy, and the observation group received mechanical ventilation on the basis of the control group. Observation and comparison were made on respiratory rate, heart rate, blood pressure, p H, arterial oxygen pressure, arterial carbon dioxide partial pressure, oxygen saturation, oxygenation index between the two groups. Results After treatment, both groups had lower respiratory rate, heart rate and blood pressure than before treatment, and the difference was statistically significant(P〈0.05). After treatment, the observation group had significantly lower respiratory rate, heart rate and blood pressure than the control group, and the difference was statistically significant(P〈0.05). After treatment, both groups had significantly higher p H, arterial oxygen pressure, oxygen saturation, oxygenation index than before treatment, and lower arterial carbon dioxide partial pressure than before treatment. Their difference was statistically significant(P〈0.05). After treatment, the observation group had significantly higher p H, arterial oxygen pressure, oxygen saturation, oxygenation index than the control group, and lower arterial carbon dioxide partial pressure than the control group. Their difference was statistically significant(P〈0.05). After 5 days of treatment, both groups had obviously lower plasma endothelin-1(ET-1) level than before treatment, and the difference was statistically significant(P〈0.05). After 5 days of treatment, the observation group had obviously lower ET-1 as(40.09±10.36) μmol/L than(49.13±9.75) μmol/L in the control group, and their difference was statistically significant(P〈0.0
