摘要
目的探讨血浆脑钠肽(B-type natriuretic peptide,BNP)浓度与脓毒症患儿病情的相关性及其对预后的预测作用,为脓毒症的诊疗提供参考。方法选取2012年1月至2016年12月于本院PICU接受治疗的244例脓毒症患儿作为研究对象,入院24 h内检测血浆BNP浓度,分析血浆脑钠肽浓度与病情的相关性,及其对预后的预测作用。结果不同严重程度脓毒症患儿,血浆BNP浓度差异有统计学意义(F=28.773,P=0.000);不同预后患儿血浆BNP浓度差异有统计学意义(F=15.821,P=0.000);血浆BNP浓度与结局的严重程度成正相关。以血浆BNP浓度预测脓毒症患儿预后,敏感度为87.9%,特异度为65.4%,ROC曲线下面积AUC=0.793,95%CI(0.706,0.881),P=0.000。结论血浆BNP浓度与脓毒症患儿病情相关,对脓毒症患儿预后有一定预测作用,应与其他指标联合检测以提高预测的敏感度和特异度。
Objective To investigate the correlation between plasma brain natriuretic peptide（ B-type natriuretic peptide,BNP） concentration and disease situation of sepsis in children with the disease and the predict effects of prognosis,and provide a reference for the diagnosis and treatment of sepsis. Methods 244 Cases of children with sepsis,from January 2012 to December 2016,in PICU of our hospital were treated as the research object,within 24 h after admission,plasma BNP concentrations were determinate,the correlation between plasma brain natriuretic peptide concentration and severity of sepsis was analyzed,as well as the predictive effect of prognosis.Results There were statistically significant differences in plasma concentrations of BNP among children with different severity of sepsis（ f = 28. 773,P = 0. 000）. The difference of plasma concentration of BNP was statistically significant in children with different prognosis（ f = 15.821,P = 0.000）. The prediction outcome from plasma BNP concentration for the prognosis,the sensitivity was 87.9%,specificity was 65.4%,the area under the ROC curve AUC = 0. 793,95% CI（ 0. 706,0. 881,P = 0. 000）. Conclusions Plasma BNP concentration is associated with the severity of sepsis in children,and has a certain predictive effect on the prognosis of children with sepsis. It should be combined with other indicators to improve the sensitivity and specificity of the prediction.
