摘要
考察雷丰《时病论》涉及时病误治原因的理论探讨及辨误救治的医案,对其时病误治原因的认识及挽救之法进行研究。雷丰认为时病失治误治的原因,主要是未能细致考察时令、体质、因机、病位、病势、病程等因素,导致辨证、治法、药量等错误。雷丰强调在治疗时首先需根据时令气候等辨其病证,注意邪正进退,应以祛邪为主、补正为辅,切忌误补,但针对危重病、久病、病体虚衰时也需考虑补正。雷丰指出勿过用药物,过犹不及易损正气,注意饮食禁忌,以防损药效、助病邪、伤病体。最后,雷丰对当时"俗医以伪混真"的恶劣医风进行批驳。总的来看,雷丰充分考虑时令特点和患者体质等因素,提出失治误治的挽救之法,其记录的救误案例及对时病正邪关系的判断,对后世总结临床经验,吸取误治教训,具有一定借鉴价值。
Study on the causes of mistreatment and remedy of exogenous diseases of Treatise on Seasonal Diseases. Lei Feng believed that the main reason of mistreatment of exogenous diseases was no comprehensive consideration of the following factors,such as seasonal climate,constitution of the patients,etiology and pathogenesis,location and duration of disease.Lei Feng emphasized seasonal characteristics and healthy energy-evil struggle should be considered during differentiation and treatment,eliminating pathogenic factors was main treatment method and strengthening vital qi as supplementary means for exogenous diseases. The invigorator misuse should be avoided.He pointed out the doctors should pay attention to abuse of medication and nutrition taboos to prevent lower efficacy,encouraging evil,damaging vital qi. He criticized the medical morals of mixing sprious with genuine. Overall,Lei Feng′s study on the causes of mistreatment and remedy of exogenous disease,can be used for reference in summing up clinical experience,drawing lessons from mistreatment for later physicians.
