CEA、CYFRA21-1、NSE、CA125联合检测在肺癌诊断中的应用效果 被引量:20

摘要 目的综合分析CEA(血清癌胚抗原)、CYFRA21-1(细胞角蛋白19片段)、NSE(神经元特异性烯醇化酶)、CA125(糖类抗原125)联合检测在肺癌诊断中的应用效果。方法选取180例肺癌(60例鳞癌、60例腺癌、60例小细胞癌)患者临床资料作为研究组,再选取同期60例健康志愿者作为对照组。采用电化学发光法检测研究对象的血清癌胚抗原水平、细胞角蛋白19片段水平、神经元特异性烯醇化酶水平、糖类抗原125水平以及分析4种肿瘤标志物联合检测肺癌的敏感性、特异性等参数指标。结果 (1)研究组的血清癌胚抗原水平、细胞角蛋白19片段水平、神经元特异性烯醇化酶水平、糖类抗原125水平与对照组比较,差异有统计学意义(P<0.05)。(2)腺癌组与鳞癌组、小细胞肺癌组血清癌胚抗原水平比较,差异有统计学意义(P<0.05);鳞癌组与小细胞癌、腺癌组细胞角蛋白19片段水平比较,差异有统计学意义(P<0.05);小细胞癌组与鳞癌组、腺癌组神经元特异性烯醇化酶水平比较,差异有统计学意义(P<0.05)。(3)研究组Ⅲ~Ⅳ期患者CEA、CYFRA21-1、NSE、CA125水平均显著高于Ⅰ~Ⅱ期患者(P<0.05)。(4)4种肿瘤标志物联合检测肺癌的敏感性远远高于任何一种肿瘤标志物的单项检测(P<0.05)。结论 4种肿瘤标志物联合检测在肺癌诊断和临床分期中有一定价值。血清癌胚抗原对肺腺癌、细胞角蛋白19片段对肺鳞癌,神经元特异性烯醇化酶对肺小细胞癌的诊断价值比较高。 Objective To investigate the effect of CEA (serum carcinoembryonic antigen), CYFRA21-1 (cytokeratin 19 fragment), NSE (neuron-specific enolase) and CA125 (carbohydrate antigen 125 ) in the diagnosis of lung cancer. Methods The clinical data of 180 patients with lung cancer (60 cases of squamous cell carcinoma, 60 cases of adenocarcinoma and 60 cases of small cell carcinoma) were selected as the study group, and 60 healthy volunteers were selected as the control group. The levels of CEA ,CYFRA21-1 ,NSE and CA125 and the sensitivity and specificity of 4 tumor markers in the diagnosis of lung cancer were detected by electrochemil method and SPSS20.0 statistical software. Results ① The serum carcinoembryonic anti- gen level, cytokeratin 19 fragment level, neuron specific enolase level and carbohydrate antigen 125 level in the study group were significantly different from those of the control group ( P 〈 0.05 ). ②The levels of serum carcinoembryonic antigen and cytokeratin 19 were significantly different between adenocarcinoma group and squamous cell carcinoma group (P 〈 0.05 ). The levels of neu- ron-specific enolase and cytokeratin 19 fragment between small cell carcinoma group and squamous cell carcinoma group were sig- nificantly different (P 〈 0. 05 ). The level of neuron specific enolase and the level of serum carcinoembryonic antigen between small cell carcinoma group and adenocarcinoma group were significantly different ( P 〈 0.05 ).③CEA,CYFRA21-1 ,NSE,CA125 levels of stage III - IV patients were significantly higher than stage I - II patients in the study group. ④The sensitivity of the 4 kinds of tumor markers in the diagnosis of lung cancer was much higher than that of any tumor marker ( P 〈 0. 05 ). Conclusion 4 kinds of tumor markers have certain value in the diagnosis and clinical staging of lung cancer.
