摘要
目的通过动态观察极低出生体重儿(VLBW)的骨代谢指标,了解VLBW的骨代谢特点,为VLBW代谢性骨病的防治提供依据。方法选择NICU的VLBW作为研究组,另选同期入住NICU的足月新生儿作为对照组,分别于出生后第1天和第22天时采血测定甲状旁腺激素(PTH)、骨钙素、25(OH)维生素D、血清钙、血清磷、碱性磷酸酶,分析骨代谢指标的变化情况。结果生后第1天研究组血清Ca、P、25(OH)Vit D水平显著低于对照组,2组比较差异有统计学意义(P<0.05),而血清OC、ALP、PTH水平2组比较差异无统计学意义(P>0.05)。生后第22天研究组血清Ca、P、25(OH)维生素D水平仍然显著低于对照组(P<0.05),血清ALP、PTH水平显著高于对照组(P<0.05)。2组婴儿血清OC水平比较差异无统计学意义(P>0.05)。结论 VLBW出生时钙磷及维生素D储备不足,骨代谢活跃,易发生骨代谢指标的异常,应及时补充钙、磷等矿物质及维生素D,以预防代谢性骨病。
Objective To compare the levels of serum bone metabolism index in preterm infants with very low birth weight (VLBW), and to provide evidence for prevention of metabolism bone disease in VLBW. Methods 120 cases of VLBW hospitalized in our NICU were selected as the case group, and another 120 cases of full term infant as control group. The serum levels of calcium, phos- phorus, alkaline phosphatase, 25 (OH) Vitamin D, osteocalcin, parathyroid hormone were detected at the first day and the 22th day after their birth. Results At the first day after birth the serum levels of calcium, phosphorus, 25 (OH)Vitamin D was extremly lower in the case group than these in control group (P 〈 0.05). At the fourth week after birth the serum levels of calcium, phosphorus, 25 (OH)Vi- tamin D was steel extremly lowerin the case group than these in control group, and the serum levels of alkaline phosphatase, parathyroid hormone was extremly high than these in control group (P 〈 0.05 ). Conclusion Insufficient calcium, phosphorus, 25 (OH) Vitamin D reserves for VLBW. Bone metabolism of VLBW is actively and bone metabolism index is often abnormality. The serum levels of calcium, phosphorus, alkaline phnsphatase, 25 (OH)Vitamin D are sensitive index for early diagnose of bone metabolism disease of VLBW.
