摘要
目的:对江苏省实施了公立医院综合改革的医院进行效果评价研究,在了解其改革成效及进展的基础上,提出深化推进改革的建议。方法:根据江苏省公立医院改革的现状,构建由运行效率、内部管理和社会效益三个方面(八个角度共24个指标)组成的评价指标体系,收集整理实施改革的江苏省公立医院在2008~2015年的数据,运用间断时间序列分析方法分析医院改革前后诊疗人次、住院天数、人均门诊住院费用、总收入等数据的变化情况。结果:目前的公立医院改革对医院的运行效率、内部管理和社会效益产生了一定的影响,改革效果初步呈现,医院运行总体平稳,费用增长幅度有所控制;但以药补医机制仍然存在,未能彻底实现提高医院运行效率、减轻患者医疗负担的目的。结论:公立医院要进一步完善和推进改革的实施,做到理顺医疗服务价格体系,落实完善政府的投入责任、重视和优化资源配置公平,完善公立医院监管体制和协同推进相关改革措施。
Objective To study the effect of comprehensive reform on public hospitals in Jiangsu Province, and put forward countermeasures. Methods Evaluation index system composed of operation efficiency, internal management and social benefit was developed. Data of Jiangsu Provincial Public Hospital from 2008 to 2015 was collected and studied with intermittent time series a- nalysis. Results The current reform on public hospitals has certain positive influence on their operation efficiency, internal man- agement and social benefits. However, the reform still needs to be improved in increasing hospitals＇ operating efficiency and re- ducing the patients＇ medical burden. Conclusion It is suggested for the public hospitals to further improve and promote the reform, straighten out the medical service price system, improve the governmental responsibility, optimize the resources allocation, perfect the public hospitals＇ supervision system and promote the relevant reform measures.
