摘要
目的 :探讨毛囊蠕形螨感染与皮肤病及皮肤肿瘤的关系。方法 :回顾性研究本院近 2年来的皮肤活检病理资料和乳腺癌根治手术标本的乳头 ,统计分析毛囊蠕形螨感染的好发部位、年龄以及与各种皮肤病、肿瘤的关系。 结果 :毛囊蠕形螨各部位感染率依次为乳头 (48.89%,6 6 / 135 ) >头面部 (2 9.2 1%,5 2 / 178) >外阴 (17.14 %,6 / 35 ) >其他部位 (4.4 2 %,8/ 181) ;各年龄段感染率 :2 0～ 2 9岁为 6 .76 %(5 / 74 ) ,30～ 39岁为 9.2 1%(7/ 76 ) ,4 0～ 4 9岁为 16 .4 7%(14 / 85 ) ,5 0～ 5 9岁为 2 3.33%(14 / 6 0 ) ,6 0～ 70岁 2 6 .2 6 %(2 6 / 99) ;各皮肤病的感染率 :上皮性肿瘤的感染率最高 5 6 .4 1%>(2 2 / 39) ,其后依次是瘤样病变18.5 8%(34/ 183)、软组织肿瘤 9.86 %(7/ 71)、非赘生性病变 2 .97%(3/ 10 1)。而上皮性肿瘤中又以基底细胞癌和外毛根鞘瘤蠕形螨的感染率为最高 (72 .72 %,16 / 2 2 )。结论 :本组资料表明 ,(1)乳头是毛囊蠕形螨感染发生率最高的部位 ,而非以往认为的头面部皮肤是毛囊蠕形螨最易感染的部位 ;(2 )毛囊蠕形螨感染与年龄有关 ,随年龄的增长感染机会增加 ;(3)上皮性肿瘤病变中毛囊蠕形螨的感染率最高 。
Objective:To investigate the relationship between demodex infestation and dermatosis, skin neoplasms.Methods:The mites prevalence, infested sites, age of patients (in skin biopsies) and the nipples obtained from patients with different skin lesions or breast excision in the last 2 years were studied. Results:The prevalence in nipple (48.89%, 66/135) was significantly higher than other part of body[facial skin (29.21%,52/178),vulva (17.14%, 6/35) and other sites(4.42%,8/181)]. The prevalence of age group from 60 70 years (26.26%,26/99) was higher than those of the others.The lowest one was in age group from 20 29 years (6.76%,5/74).The mite prevalence(56.41%,22/39) in epithelial tumor was significantly higher than tumor like disease group (18.58%,34/183), soft tissue tumor group (9.86%,7/71) and non tumor lesion group (2.97%,3/101). The highest mite prevalence in the epithelial tumor group was basaloma and trichilemmoma sub group (72.72%, 16/22). Conclusion:The most infested areas of demodex mites is facial skin in the past,while our conclusion is the nipple. The prevalence of the mite increases with age. In this study the highest mite prevalence is in the epithelial tumor group,especially in basaloma and trichilemmoma.
