摘要
目的分析六味地黄丸联合脉冲射频治疗腰椎间盘突出症(LDH)术后慢性腰腿疼痛的临床效果。方法选取85例LDH术后遗留慢性腰腿疼痛患者,按随机数表法分为实验组(n=45)和对照组(n=40),对照组患者给予脉冲射频治疗,实验组在对照组基础上联用六味地黄丸治疗。治疗结束后进行临床疗效评定,并评估两组腰腿疼痛、功能恢复及生活质量。结果实验组临床治疗总有效率为91.11%与对照组的75.00%相比有显著性差异(P<0.05)。两组治疗前视觉模拟疼痛(VAS)评分、日本骨科学会腰痛疾患评定表(JOA)评分比较无统计学意义(P>0.05),治疗后2周、4周VAS评分、JOA评分均明显改善(P<0.05),且与对照组相比,实验组治疗后2周、4周VAS评分显著较低,JOA评分显著较高(P<0.05)。实验组治疗后4周生活质量核心量表(QOL-C30)各项评分(社会功能、心理功能、躯体功能、物质生活)均显著高于对照组(P<0.05)。结论六味地黄丸联合脉冲射频治疗LDH术后慢性腰腿疼痛的临床疗效较为理想,可明显改善患者术后疼痛及腰部功能,提高患者生活质量。
Objective To analyze the clinical effect of Liuwei Dihuang pill combined with pulsed radiofrequency in the treatment of chronic lumbar and leg pain after lumbar disc herniation（LDH）surgery. Methods A total of 85 patients with chronic lumbar and leg pain after LDH surgery wereselected, and randomly divided into the experimental group（n=45） and the control group（n=40）. The control group was treated with pulsed radiofrequency, while the experimental group was given Liuwei Dihuang pill on the basis of the control group. After treatment, the clinical effect, relief of lumbar and leg pain, functional recovery and quality of life were assessed. Results There was significant difference in total effective rate between the two groups（91.11% vs 75.00%, P〈0.05）. There was no significant difference in scores of pain visual analogue score（VAS） and Japanese Orthopaedic Association（JOA） of lumber pain between the two groups before treatment（P〈0.05）. 2 and 4 weeks after treatment, VAS scores and JOA scores were significantly improved（P〈0.05）, and compared with the control group, the VAS score was significantly lower and JOA score was significantly higher in the experimental group（P〈0.05）. The scores of quality of life core scale（QOL-C30）（social function,mental function, physical function and material life） in the experimental group were significantly higher than those in the control group at 4 weeks after treatment（P〈0.05）. Conclusion Liuwei Dihuang pill combined with pulsed radiofrequency is effective in the treatment of chronic lumbar and leg pain after LDH surgery. The treatment can significantly relieve postoperative pain and lumbar function, and improve their quality of life.
