摘要
目的:探讨卡贝缩宫素联合缩宫素预防人工流产后出血临床价值。方法:收集2010年3月至2015年2月共146例人工流产患者临床资料,按住院号单双平均分成两组,对照组73例,米索前列醇联合米非司酮常规治疗,并子宫壁肌注缩宫素20μL,观察组在对照组基础上卡贝缩宫素20U加入500mL平衡液中静脉滴注,观察不同方法治疗后出血情况、月经恢复情况、基础体温变化情况比较。结果:观察组阴道出血时间<3d的比率为52.05%,显著高于对照组的10.96%(P<0.05);对照组阴道出血平均时间为(7.56±1.43)d、出血量为(52.74±9.75)mL,观察组则为(4.25±1.01)d、出血量为(41.45±4.56)mL;观察组月经恢复正常时间<28d比率为58.9%显著优于对照组的39.73%(P<0.05),对照组月经恢复正常平均时间(39.74±10.46)d,观察组为(30.65±4.53)d,观察组显著优于对照组(P<0.05);两组在基础体温下降时间、排卵时间、黄体期时间上无统计学意义(P>0.05)。结论:卡贝缩宫素联合缩宫素能减少人工流产后出血量和出血时间,月经恢复时间较短。
Objectives： To investigate the clinical value of Hemorrhage after abortion in the prevention of hemorrhage after abortion. Methods： 146 cases of artificial abortion from March 2010 to February 2015 were divided into 2 groups,with 73 cases in each group. The control group received mifepristone and misoprostol combined with conventional treatment,and intramuscular injection of oxytocin 20 uL in uterine wall. On the basis of control group＇s treatment,the observation group received carbetocin 20 u joined 500 ml intravenous injection in equilibrium.The bleeding,menstrual recovery and basal body temperature changes after abortion in the two groups were compared. Results： In observation group,vaginal bleeding time 3 d rate was 52. 05%,the rate of 3-7 d was 41. 1%and the rate of 7 d was 6. 85%,which were significantly higher than these in control group（ P〈0. 05）. The average time of vaginal bleeding of control group was（ 7. 56 ± 1. 43） d and HP（ 52. 74 ± 9. 75） ml,and the observation group was（ 4. 25 ± 1. 01） d and（ 41. 45 ± 4. 56） ml. In observation group,menstrual recovery time 28 d rate was 58. 9% and the rate 38 d was 2. 74%,which were significantly higher than these in control group（ P〈0. 05）. The average recovery time to normal menstruation in control group was（ 39. 74 ± 10. 46） d,and it was（ 30. 65 ± 4. 53） d in observation group. The observation group was significantly better than control group（ P〈0. 05）. The fall time of basal body temperature,the time of ovulation and luteal phase time of two groups had no statistically significant difference（ P〈0. 05）. Conclusion： Carbetocin combined with oxytocin can reduce the amount of bleeding after abortion and bleeding time,with short menstrual recovery time.
