摘要
目的:探讨两种术前不同放化疗方案在Ⅱb期宫颈癌患者中的临床效果。方法:取2014年12月至2016年12月我院收治Ⅱb期宫颈癌患者100例,随机数字法分为对照组(n=50)和观察组(n=50)。两组均行宫颈癌切除术治疗,对照组术前采用5-氟尿嘧啶+顺铂联合同步放化疗治疗,观察组术前采用博来霉素、长春新碱、顺铂联合同步放化疗治疗,比较两组临床疗效。结果:两组放化疗前体积比较差异无统计学意义(P<0.05);观察组放化疗后体积、手术时间、术中出血量均少于对照组(P<0.05);观察组治疗后白细胞减少、血小板减少、恶心呕吐、肝功能损伤等毒副反应发生率低于对照组(P<0.05);观察组治疗后1年、3年及5年生存率高于对照组(P<0.05)。结论:Ⅱb期宫颈癌患者术前采用博来霉素、长春新碱、顺铂联合同步放化疗治疗效果理想,利于手术切除,能降低毒副反应发生率,值得推广应用。
Objectives: To investigate the clinical effect of two preoperative radiotherapy and chemotherapy regimens in patients with stage IIb of cervical cancer. Methods: From December 2014 to December 2016,100 patients with cervical cancer IIb were randomly divided into control group( n = 50) and observation group( n = 50).Patients in both groups were treated with cervical cancer surgical resection,the control group before the use of 5-fluorouracil and cisplatin combined with concurrent chemoradiotherapy,observation group before the use of bleomycin,vincristine,cisplatin combined with concurrent radiotherapy and chemotherapy. The clinical efficacy of the two groups was compared. Results: There was no significant difference in the volume of the two groups between before and after chemotherapy( P〈0. 05). The volume,operation time and intraoperative blood loss were significantly lower in the observation group than that in the control group( P〈0. 05). The 1-year,3-year and 5-year survival rate of the observation group were higher than those of the control group( P〈0. 05). The incidence of side effects such as leukopenia,thrombocytopenia,nausea and vomiting and liver injury was lower than that of the control group( P〈0. 05). Conclusion: The efficacy of ibuprofen,vincristine and cisplatin combined with concurrent chemoradiation is ideal for patients with cervical cancer IIb. It is beneficial to surgical resection and reduce the incidence of toxicity,thusly worthy of popularization and application.
