摘要
目的研究血清降钙素原(PCT)、C-反应蛋白(CRP)在下呼吸道细菌感染诊断中的意义,提高临床诊治水平。方法选取2016年1月至2017年1月山西博爱医院收治的下呼吸道感染患者100例,按临床诊断分为细菌感染组50例和非细菌感染组50例,并选同时间段内健康体检者50例为对照组。采用双抗体夹心免疫层析法测定PCT水平,采用免疫比浊法测定CRP水平,所有患者行痰液细菌培养,统计对比三组的血清PCT、CRP水平和阳性率,统计感染组的细菌培养检出率与菌株分布情况。结果细菌感染组PCT水平明显高于非细菌感染组、对照组,组间比较差异有统计学意义(P<0.05);非细菌感染组PCT水平与对照组比较差异无统计学意义(P>0.05);细菌感染组CRP水平与非细菌感染组比较,差异无统计学意义(P>0.05),与对照组比较差异有统计学意义(P<0.05)。细菌感染患者中检病原体检出率52%,非细菌感染患者中病原体检出率34%,对比差异有统计学意义(P<0.05)。结论 PCT为细菌感染的重要血清学指标,CRP升高是急性感染时的重要血清学指标,但是不能区分感染是否是细菌所致,两者相结合才能提高确诊率。
Objective To study the diagnostic significance of serum PCT and CRP in bacterial infection in lower respiratory tract and improve the clinical diagnosis and treatment. Methods Between January 2016 and January 2016 of our hospital,100 patients with lower respiratory tract infections,according to the clinical diagnosis is divided into bacterial infection group of 50 cases and the bacterial infection group of 50 cases,and choose with period health check-up 50 cases as control group. With double antibody sandwich immune chromatography determination of PCT level,the immune turbidimetric method determination of CRP levels,all patients sputum bacterial culture,statistics compared three groups of serum PCT and CRP levels and positive rate,statistical group infection rate of bacterial culture and strain distribution.Results The PCT level in bacterial infection group was obviously higher than that of the bacterial infection group and the control group,comparing differences between groups was statistically significant( P 〈 0. 05); The PCT level in bacterial infection group there was no statistically significant difference compared with control group( P 〉 0. 05); Bacterial infection group of CRP level compared with the bacterial infection group,there was no statistically significant difference( P 〉 0. 05),compared with the control group was statistically significant difference( P 〈 0. 05). Bacterial infection in patients with pathogen detection rate of 52% ,the bacterial infection in patients with pathogen detection rate 34% ,compare the difference was statistically significant( P 〈 0. 05). Conclusion PCT important serological indexes for bacterial infections,elevated CRP is important serological indexes during acute infection,but cannot distinguish whether infections caused by bacteria,the combination can improve the diagnosis rate.
