摘要
目的 探讨经尿道等离子电切联合球囊扩张筋膜扩张器治疗外伤性前尿道狭窄的临床疗效.方法 选取80例外伤性前尿道狭窄患者,随机分为两组,观察组和对照组各40例.观察组采用等离子电切联合球囊扩张筋膜扩张器电切术,对照组采用等离子电切术.观察两组患者的一般临床资料、手术时间、术中出血量、术后下床活动时间、留置尿管时间、术后住院时间、术后并发症、平均尿流率、膀胱残余尿量等.结果 两组患者一般临床资料包括年龄、病程、平均最大尿流率、平均膀胱残余尿余量、狭窄长度等,差异均无统计学意义（P＞0.05）.在手术时间、出血量、术后下床活动时间、留置尿管时间等方面,经统计学分析,差异均不具有统计学意义（P＞0.05）.两组在平均尿流率、残余尿量等方面,差异均具有统计学意义（P＜0.05）;在住院时间、术后并发症等方面,差异均不具有统计学意义（P＞0.05）.结论 经尿道等离子电切联合球囊扩张筋膜扩张器治疗外伤性前尿道狭窄操作简便、安全有效.可加速患者临床康复,减少术后并发症的发生,患者痛苦小、成功率高,值得在临床上进行推广应用.
Objective To investigate the clinical application of the transurethral plasma electrocision with the balloon catheter in the treatment of traumatic anterior urethral stricture.Methods A total of 80 patients with urethral stricture were randomized into two groups：the observation group undergoing the transurethral plasma electrocision with the balloon catheter and the control group undergoing the transurethral plasma electrocision.Operating time,transoperative bleeding,postoperative out-of-bed activity time,detain ureter time,postoperative length of stay,postoperative complication,average flow rate,bladder residual urine volume and so forth were compared between the two groups.Results There were no significant differences in general clinical data,include age,pathogenesis,average flow rate,bladder residual urine volume,stenosis length between the two groups （P ＞ 0.05）.In intraoperative condition,compared with the control group,operating time,bleeding,transoperative bleeding,postoperative out-of-bed activity time,detain ureter time were not significantly different （P ＞ 0.05）.After operation,statistically significant differences were found for average flow rate,bladder residual urine volume （P ＜ 0.05）,while that in postoperative length of stay and postoperative complications were not significantly different （P ＞ 0.05）.Conclusions The application of the transurethral plasma electrocision with the balloon catheter is operating convenient,safety and effectiveness for the treatment of traumatic anterior urethral stricture.It can accelerate clinical recovery,decrease the rate of postoperative complications,less pain and high success rate,be thus worthy of promotion and application.
