摘要
目的观察经尿道腔内技术治疗复杂性尿道狭窄患者的临床疗效。方法对我院于2015年3月至2017年3月收治的58例经尿道腔内技术治疗的复杂性尿道狭窄患者的临床资料进行回顾性分析,所有患者均经过尿道镜、B超等检查诊断为复杂性尿道狭窄。所有患者均采用绿激光汽化技术、等离子体双极电切术或冷刀行窥视下经尿道腔内切开术切除尿道瘢痕组织的治疗方法进行治疗。结果58例患者中,1次手术成功患者46例,1次手术成功率为79.31%;2~3次手术成功患者7例,2~3次手术成功率为12.07%;5例患者腔内手术失败,手术失败率为8.62%。患者手术失败的原因包括两断端严重移位、尿道闭锁段过长、尿道狭窄、骨盆骨折畸形愈合严重压迫尿道、术后尿道感染等。对患者进行术后随访,时间为6个月,共计47例患者平均最大尿流率>15 mL/s,排尿通畅率为81.03%。结论在治疗复杂性尿道狭窄患者过程中运用经尿道腔内技术具有显著的临床效果。
Objective To observe the clinical efficacy of transurethral technique in the treatment of patients with complicated urethral stricture.Methods The clinical data of 58 patients with complicated urethral stricture treated by endourethral technique treated in our hospital from March 2015 to March 2017 were retrospectively analyzed.All patients had undergone urethroscopy and B-ultrasound examinations and were diagnosed as complicated urethral stricture.All patients were treated with green laser vaporization technology,plasma bipolar resection or cold knife transurethral incision under peeping to remove urethral scar tissue.Results Among the 58 patients,46 patients had one successful operation,and the success rate of one operation was 79.31%;7 patients were successful in 2-3 operations,and the success rate of 2-3 operations was 12.07%;5 patients had intracavitary surgery failure,the operation failure rate was 8.62%.The reasons for the failure of the patient's operation include severe displacement of the two ends,excessively long urethral atresia,urethral stricture,malunion of pelvic fracture severely presure on the urethra,postoperative urethral infection,etc.The patients were followed up for 6 months after surgery.A total of 47 patients had an average maximum urine flow rate of more than 15 mL/s,and a patency rate of 81.03%.Conclusion The application of transurethral technique in the treatment of patients with complex urethral stricture has a significant clinical effect.
