摘要
目的探讨子痫前期(PE)孕妇子宫螺旋动脉超声参数与胎盘组织中血管新生、细胞凋亡分子含量的相关性,为临床治疗提供参考。方法选择2013年5月-2016年10月期间在该医院诊断为PE的38例孕妇作为PE组,选择同期分娩的60例健康孕妇作为对照组。孕33~38周时采用彩色多普勒超声诊断仪测定子宫螺旋动脉的血流参数,分娩后收集胎盘组织并测定血管新生、细胞凋亡相关分子的含量。结果孕33~38周时,PE组孕妇子宫螺旋动脉的搏动指数(PI)、阻力指数(RI)和收缩末期峰值流速/舒张末期流速(S/D)比值均显著高于对照组,差异有统计学意义(均P<0.05);分娩后,PE组胎盘组织中血管内皮生长因子A(VEGFA)、VEGFB、胎盘生长因子(PLGF)、血管内皮生长因子受体-1(VEGFR-1)、VEGFR-2、生存素(Survivin)、X连锁凋亡抑制蛋白(XIAP)、B淋巴细胞瘤-2基因(Bcl-2)、Livin的含量显著低于对照组,自杀相关因子(Fas)、Fas配体(Fas L)、Bcl-2相关X蛋白(Bax)、含半胱氨酸的天冬氨酸蛋白水解酶-3(Caspase-3)、Caspase-8、Caspase-9的含量显著高于对照组,差异有统计学意义(均P<0.05);PI、RI、S/D比值与胎盘中VEGFA、VEGFB、PLGF、VEGFR-1、VEGFR-2、Survivin、XIAP、Bcl-2、Livin的含量呈负相关关系,与胎盘中Fas、Fas L、Bax、Caspase-3、Caspase-8、Caspase-9的含量呈正相关关系(均P<0.05)。结论 PE孕妇子宫螺旋动脉的阻力显著增加并且与胎盘中血管新生、细胞凋亡密切相关。
Objective To explore the correlations between ultrasonic parameters of uterine spiral artery and angiogenesis,apoptosis in placenta of pregnant women with preeclampsia,provide a reference for clinical treatment. Methods Thirty-eight pregnant women diagnosed as preeclampsia in the hospital from May 2013 to October 2016 were selected as preeclampsia group,and 60 healthy pregnant women who delivered during the same period were selected as control group. The blood flow parameters of uterine spiral artery were measured by color Doppler ultrasound at 33-38 weeks of gestation. The placental tissue samples were collected after delivery,the levels of related molecules of angiogenesis and apoptosis were measured. Results At 33-38 weeks of gestation,pulsatility index( PI) and resistance index( RI) of uterine spiral artery,and the ratio of peak systolic velocityto end diastolic velocity( S/D) in preeclampsia group were statistically significantly higher than those in control group( all P〈0. 05). After delivery,the levels of vascular endothelial growth factor A( VEGFA),vascular endothelial growth factor B( VEGFB),placental growth factor( PLGF),vascular endothelial growth factor receptor-1( VEGFR-1),vascular endothelial growth factor receptor-2( VEGFR-2),Survivin,X-linked inhibitor of apoptosis protein( XIAP),B cell lymphoma-2 gene( Bcl-2),and Livin in placental tissues of preeclampsia group were statistically significantly lower than those of control group( all P〈0. 05). The levels of Fas,Fas ligand( Fas L),Bcl-2 associated X protein( Bax),Caspase-3,Caspase-8,and Caspase-9 in placental tissues of preeclampsia group were statistically significantly higher than those of control group( all P〈0. 05). PI,RI,and S/D were negatively correlated with the levels of placental VEGFA,VEGFB,PLGF,VEGFR-1,VEGFR-2,Survivin,XIAP,Bcl-2,and Livin( all P〈0. 05). PI,RI,and S/D were positively correlated with the levels of placental Fas,Fas L,Bax,Caspase-3,Caspase-8,and Caspase
