摘要
目的分析髋关节置换术对股骨颈骨折老年患者的治疗效果。方法 54例老年股骨颈骨折患者,按照治疗方式不同分为实验组与参照组,各27例。实验组采用髋关节置换术治疗,参照组采用骨折内固定术治疗,比较两组患者的临床症状恢复情况以及自主生活能力转变情况。结果参照组治疗前髋关节功能评分标准(Harris)评分为(15.56±3.77)分,治疗1个月为(28.04±1.65)分,治疗3个月为(54.18±1.55)分;实验组治疗前Harris评分为(15.57±3.49)分,治疗1个月为(43.12±2.43)分,治疗3个月为(74.25±2.23)分;治疗后1、3个月两组患者的Harris评分均高于治疗前,且实验组高于参照组,差异具有统计学意义(P<0.05)。实验组患者的各项巴塞尔指数(Barthel)指数评分均高于参照组,差异具有统计学意义(P<0.05)。结论髋关节置换术能够有效改善老年股骨颈骨折患者的临床症状,提升其自主生活能力,对老年股骨颈骨折患者生活质量的提升能够产生积极影响。
Objective To analyze the treatment effect of hip arthroplasty for the senile patients with femoral neck fracture. Methods A total of 54 senile patients with femoral neck fracture were divided by different treatment methods into experimental group and control group, with 27 cases in each group. The experimental group was treated with hip arthroplasty, and the control group was treated with internal fixation of fracture. Comparison were made on recovery of clinical symptoms and the change of independent living ability between two groups. Results The control group had Harris hip score before treatment as （15.56 ± 3.77） points, （28.04 ± 1.65） points after 1 month of treatment and （54.18±1.55） points after 3 months of treatment, while the experimental group had Harris score before treatment as （15.57 ±3.49） points, （43.12 ±2.43） points after 1 month of treatment and （74.25 ± 2.23） points after 3 months of treatment. After 1 and 3 months of treatment, both groups had higher Harris score than before treatmenL and the experimental group was higher than the control group. Their difference was statistically significant （P〈0.05）. The experimental group had higher Barthel index scores than the control group, and the difference was statistically significant （P〈0.05）. Conclusion Hip arthroplasty can effectively improve the clinical symptoms of senile patients with femoral neck fracture, and improve their ability of independent living. This method has a positive impact on the cmalitv of life of senile oatients with femoral neck fracture.
