摘要
目的探讨高强度聚焦超声(HIFU)在子宫肌瘤治疗中的应用效果。方法选择2013年4月-2016年5月在宜宾市第二人民医院行HIFU治疗的68例子宫肌瘤患者临床资料进行回顾性分析,观察患者治疗前、后的平均肌瘤直径及平均肌瘤体积、临床症状及血清血红蛋白(HGB)、黄体生成素(LH)、雌二醇(E_2)、孕酮(P)、血清卵泡刺激素(FSH)指标水平,并对结果进行分析比较。结果治疗后,患者的平均肌瘤直径及平均肌瘤体积较治疗前均显著缩小,差异均有统计学意义(P<0.05)。患者经期过长、贫血、痛经、尿频、压迫等临床症状均得到了明显改善。患者血清HGB、LH水平显著高于治疗前,血清P、E2、FSH水平显著低于治疗前,差异均有统计学意义(P<0.05)。结论采用高强度聚焦超声治疗子宫肌瘤其疗效确切,对患者的临床症状及血清HGB、LH、P、E_2、FSH等指标均有明显的改善作用,且操作简便、安全性好,可在临床中推广应用。
Objective To explore the application effect of high-intensity focused ultrasound (HIFU) in treatment of uterine fibroids. Methods The clinical data of 68 patients with uterine fibroids treated by HIFU in the Second People's Hospital of Yibin from April 2013 to May 2016 was analyzed retrospectively. The mean diameters and volumes of uterine fibroids, clinical symptoms, and levels of serum hemoglobin ( HGB), luteinizing hormone ( LH), estradiol ( E2 ), progesterone ( P), and follicle-stimulating hormone ( FSH ) before and after treatment were observed, then the results were analyzed and compared. Results The mean diameters and volumes of uterine fibroids after treatment were statistically significantly less than those before treatment ( P〈0. 05 ) . After treatment, the clinical symptomes of the patients were improved significantly, such as menostaxis, anemia, dysmenorrhea, frequent urination, and oppression. The levels of serum HGB and LH after treatment were statistically significantly higher than those before treatment (P〈0. 05), while the levels of serum P, E2, and FSH after treatment were statistically significantly lower than those before treatment (P〈0. 05 ) . Conclusion The curative effect of HIFU in treatment of uterine fibroids is definite, HIFU can significantly improve clinical symptoms and serum indexes (HGB, LH, P, E2 , and FSH), the operation is simple, and the safety is high, which is worthy of clinical promotion and application.
