吸烟对支气管哮喘患者外周血单个核细胞CC趋化因子受体7的表达的影响及临床意义 被引量:7

摘要 通过研究吸烟对支气管哮喘 (简称哮喘)患者外周血单个核细胞 CC趋化因子受体7(CCR7)及其血清中相关配 体的表达的影响,探讨其与肺功能、血气分析等临床指 标的相关性。方法 随机选取门诊哮喘患者 (哮喘组)15例,吸烟的哮喘患者 (吸烟+哮喘组)15例,健康体检者 (对照组)15例和健康吸烟者 (吸烟组)15例。所有研究对象均进行动脉血气及肺功能的测定,哮喘组和吸烟+哮喘组患者行哮喘控制测试 (ACT)评分。采用流式细胞 仪测定外周血单个核细胞CCR7蛋白的表达,采用酶联免疫吸附试验法测定外周血 CC趋化因子配体 (CCL)19和 CCL21蛋白的表达。结果 ①与对照组比较,吸烟组、哮喘组和吸烟+哮喘组外周血单个核细胞 CCR7蛋白表达增高,差异均有统计学意义 (P 值均〈0.01);与哮喘组比较,吸烟+哮喘组外周血单个核细胞 CCR7蛋白表达增高,差异有统计学意义 (P 〈0.01);②与对照组比较,吸烟 +哮喘组外周血清中 CCL19和 CCL21蛋白表达增高,差异均有统计学意义 (P 值均〈0.01);与哮喘组比较,吸烟+哮喘组外周血清中 CCL19和 CCL21蛋白表达增高,差异均有统计学意义 (P 值均〈0.01);③与对照组比较,哮喘组和吸烟+哮喘组第1秒用力呼气容积 (FEV1)、呼气峰流速 (PEF)降低,差异均有统计学意义(P 值均〈0.01),与哮喘组比较,吸烟+哮喘组 ACT评分、FEV1 和 PEF降低,差异均有统计学意义 (P 值均〈0.05)。④相 关性分析,外周血单个核细胞CCR7蛋白的表达与FEV1、PEF呈负相关(P 值均〈0.01)。结论吸烟可以上调哮喘 患者外周血单个核细胞CCR7蛋白及其配体CCL19和CCL21蛋白的表达,这可能是吸烟使哮喘患者症状加重,肺功能下降的机制之一。 Objective To study the effect of cigarette smoke exposure on the expression of CC chemokine receptor 7 (CCRT) and its ligands of peripheral blood mononuelear cells in asthmatic patients, and to explore correlation between CCR7 and clinical targets. Methods The outpatients with asthma (asthma group, n =15) and smoking outpatients with asthma (smoking & asthma group, n =15) were enrolled in the study. Meanwhile, healthy controls (control group, n = 15) and healthy smokers (smoking group, n = 15)were matched. All of them required to blood test and assess asthma control test (ACT) score, blood gas analysis and lung function. CCR7 of peripheral blood mononuclear cells were measured by flow cytometry, CC chemokine ligand 21 (CCL21) and CCL19 in peripheral blood were measured by enzyme-linked immunosorbent assay. Results ① Compared with the control group, the expression of CCR7 protein increased remarkably in the asthma group and the smoking exposure combined asthma group (all P 〈0.01). Compared with the asthma group, the expression of CCR7 protein increased remarkably in the smoking combined asthma group (all P 〈0.01). ②Compared with the control group, CCL21 and CCL19 increased remarkably in the smoking combined asthma group (all P〈 0.01 ). Compared with the asthma group, CCL21 and CCL19 increased remarkably in the smoking combined asthma group (all P 〈0.01). ③Compared with the control group, forced expiratory volume in 1 second (FEVI) and peak expiratory flow (PEF) reduced remarkably in the asthma group and the smoking combined asthma group (all P 〈0.01). Compared with the asthma group, ACT score, FEV1 and PEF increased remarkably in the smoking combined asthma group (all P 〈0.01). ④The expression of CCR7 protein was negatively correlated with FEV1 and PEF (all P 〈0.01).Conclusions Smoking can increase the expression of CCR7 and its ligand CCL19 and CCL21, which may be result in the aggravation of symptoms and the decline of lung f
