摘要 目的使用MR扩散张量成像对正常肘部尺神经进行显示和定量评价,为临床诊断尺神经病变提供参考依据。方法对29名健康志愿者的肘关节行扩散张量成像,测量肘管不同平面尺神经的各项异性分数(fractional anisotropy,FA)、相对各向异性(relative anisotropy,RA)、容积比(volume ratio,VR)和表观扩散系数(apparent diffusion coefficient,ADC)的参数值,生成尺神经扩散示踪图。结果共分析了54个肘关节。健康人群肘管内尺神经的FA值、RA值、VR值和ADC值依次为0.373±0.054、0.324±0.053、0.857±0.045、(1.459±0.212)×10-3mm2/s。前述各项测量参数与年龄、性别无关。肘管近端与其他测量层面间尺神经的FA值、RA值、VR值存在统计学差异。同一个体内,左右侧尺神经的FA值、RA值、VR值存在统计学差异。结论正常肘部尺神经各扩散参数的定量测量值稳定可靠,可为早期诊断尺神经病变提供定量诊断依据。 Objective To display and quantitative evaluate normal diffusion values of the ulnar nerve at the elbow, so as to provide evidences for clinical diagnosis of ulnar lesion. Methods 29 healthy volunteers underwent diffusion tensor imaging (DTI) and tractography under 3. O-T MR for their elbows. The fractional anisotropies (FAs), relative anisotropies (RAs), vol- ume ratios (VRs) and apparent diffusion coefficients (ADCs) of the ulnar nerve were measured on different cross sections of the cubital tunnel. Results A total of 54 elbows were evaluated and the FAs, RAs, VRs, and ADCs of the ulnar nerve through the cubital tunnel in a healthy population were respectively 0. 373 ±0. 054, 0. 324 ±0. 053,0. 857±0. 045, ( 1. 459 ±0.212)×10^-3mm^2/s. All these parameters were not correlated with genders or ages. Statistical significance was found between FAs, RAs and VRs obtained on the proximal section and other sections of the cubital tunnel. Between the ulnar nerves in the pair of elbows from the same person, the values of the above parameters showed statistically significant differences. Conclusion Quantitative meas- uring the diffusion values of the ulnar nerve by DTI had stable and reliable results. It can be used as a reference for early diagnosis of ulnar lesions.
