摘要
目的探讨利用人工韧带重建联合自体脂肪移植进行面部悬吊提紧及容量补充对恢复面部年轻化的效果。方法从颞部发际线内入路,埋置人工韧带(可吸收性软组织提拉整形带及PPDO锯齿线),向后上方牵拉后将其末端固定于颞深筋膜层,重建颧韧带、皮肤-咬肌韧带、颊上颌韧带,提升颧脂肪垫,改善面部软组织松垂;同时于皮下多层次填充自体脂肪颗粒及纳米脂肪,改善面部软组织容量不足。结果本组18例患者获随访6个月至3年。所有患者术后面部松垂及软组织容量不足的情况均得到了不同程度地改善。患者总体满意率达83.3%,主要不满意原因为术后远期脂肪吸收。结论人工韧带重建联合自体脂肪移植术对轻中度的面部松垂和容量缺失的改善效果较好,且外观自然,患者的满意度较高,值得临床推广。
Objective To investigate the efficacy of facelift using artificial ligament combined with autologous fat transplantation in patients with facial aging. Methods Zygomatic ligament, masseteric cutaneous ligament and cheek-maxilla ligaments were rebuilt by embedding absorbable artificial ligaments（Weilamei and PPDO） into each side of the face with a small temporal in-hairline incision. The ligaments were pulled back and up to the end and then fixed in the deep temporal fascia. The malar fat pads were enhanced to improve facial soft tissue sagging. Meanwhile the autologous fat granules and nano fat were transplanted in the subcutaneous multi-level to improve facial soft tissue collapse. Results Eighteen patients were followed up from 6 months to 3 years after operation. All cases were improved to varying degrees and the degree of satisfaction was 83.3%. The main reason for dissatisfaction was the long-term absorption of fat. Conclusion This combined application of artificial ligament reestablishment and autologous fat transplantation provides a natural, balanced, rejuvenated aesthetic result with few complications and significant lifting. This procedure resulted in a high level of satisfaction among patients and surgeons. It should be considered for clinical application.
