摘要
目的探讨常规剂量和低剂量CT扫描诊断肺结节的意义。方法选择2015年1月—2017年12月本院收治的147例肺结节患者作为研究对象。本组患者均先行常规剂量扫描,再行低剂量扫描,观察低剂量和常规剂量扫描的图像技术质量,并统计两种剂量扫描检出的肺结节数量,同时对肺结节性质进行诊断。结果低剂量扫描共检出481个结节,常规剂量扫描共检出483个结节;常规剂量扫描和低剂量扫描检出≥10 mm、6~10 mm、≤5 mm结节数量及总数比较,差异无统计学意义(P>0.05)。常规剂量扫描诊断肺结节性质:肺癌率(32.71%)、肺转移瘤率(27.95%)、结核增殖灶率(31.88%)、炎性结节率(4.14%)、定性困难率(3.13%)与低剂量扫描(32.64%、27.65%、31.60%、4.37%、3.74%)比较,差异无统计学意义(P>0.05)。常规剂量和低剂量扫描图像表现均清晰,细微结构可清晰显示。结论低剂量CT扫描对肺结节诊断效果较好,与常规剂量诊断效果相似,可优先考虑低剂量CT扫描。
Objective To explore the significance of conventional and low dose CT scan in the diagnosis of pulmonary nodules. Methods 147 cases of pulmonary nodules treated in our hospital from January 2015 to December 2017 were selected as the subjects. All patients were given conventional dose scanning first, then low dose scan. The image quality of low dose and conventional dose scanning was observed. The number of pulmonary nodules detected by two dose scans was counted, and the nature of pulmonary nodules was diagnosed. Results Low dose scan detected 481 nodules, conventional dose scan detected 483 nodules; there was no statistically significant difference between conventional dose and low dose scanning about the total number of nodules over 10 mm, 6~10 mm, less than 5 mm (P 〉 0.05). Conventional dose scan for the diagnosis of pulmonary nodules: the rate of lung cancer (32.71%), the rate of tumor metastasis of pulmonary tuberculosis (27.95%), the rate of proliferation (31.88%), the rate of infammatory nodules (4.14%), the rate of qualitative difficult (3.13%), in low dose scan they were 32.64%, 27.65%, 31.60%, 4.37%, 3.74%, there was no difference statistical signifcance (P 〉 0.05). Both the conventional and low dose scanning images were clear, and the fine structure could be clearly displayed. Conclusion The low dose CT scan is better for the diagnosis of pulmonary nodules. It is similar to the conventional dose in diagnosis, and the low dose CT scan can be considered as a priority.
