一起手足口病暴发疫情的流行病学调查及危险因素分析 被引量:4

摘要 目的探讨手足口病暴发疫情的危险因素,为手足口病防制提供依据。方法利用描述性流行病学方法对疫情开展流行病学调查,采用问卷调查的方式收集资料,并用非条件logistic回归进行数据分析。结果疫情持续6 d,累计发病10例,波及1个班级,罹患率43.48%;6例病例、1例密切接触者样本经手足口病病毒核酸检测为肠道柯萨奇A组16型(Cox A16)。单因素logistic回归分析显示:是否有咬手指习惯、是否经常不洗手为手足口病发病的相关因素,将以上因素纳入多因素logistic回归模型,结果显示不经常洗手为危险因素。结论此疫情由Cox A16感染引起,密切接触可能是疫情播散的主要原因。晨检为托幼机构手足口病疫情防控的重要环节,加强幼儿手部卫生能有效预防手足口病。 Objective To investigate the risk factors of hand-foot-mouth disease( HFMD) in kindergartens,and to evaluate outbreak response measures. Methods The investigation of the HFMD outbreak was analyzed by descriptive epidemiological method. The data were collected by questionnaire and the risk factors of the outbreak were selected and analyzed by logistic regression analysis. Results The outbreak lasted for 6 d,with the overall incidence of 10 people. The patients mainly concentrated in one class,and the attack rate was 43. 48%. By the detection of virus nucleic acid,6 cases and 1 close contact were positive for Cox A16. The related factors of the outbreak were suck the finger and do not wash their hands before meals,incorporating the above factors into the multi-factors regression analysis model,the result showed that do not wash their hands before meals was the risk factor of the outbreak. Conclusion The main pathogens causing this outbreak of HFMD are Cox A16,and the possible transmission maybe the child-to-child transmission. Strengthening the morning examination in nurseries and getting into the habit of washing hands before meal are significant measures to timely discover,prevent and control HFMD.
