摘要
[背景]探讨子宫动脉介入化疗栓塞术治疗中晚期宫颈癌的疗效.[病例报告]回顾性分析2006年1月至2014年9月间行子宫动脉介入化疗栓塞术的98例中晚期宫颈癌患者的临床资料.记录术后阴道出血量及治疗3周后的病灶大小;观察不良反应、术后期治疗方式、肿瘤残留及淋巴结转移情况和5年生存率.结果见,98例中伴阴道大出血55例,72h内出血有效控制率为100.0%.手术3周后测量病灶大小结果显示,治疗总有效率为79.6%,治疗过程中均未见有严重不良反应发生.Ⅰb2~Ⅱa2期患者手术3周后可行根治术的比例为70.7%.行子宫动脉介入化疗栓塞术后淋巴结转移率为29.2%,1,3,5年生存率分别为98.3%,84.7%,71.2%.[讨论]给中晚期宫颈癌患者行子宫动脉介入化疗栓塞术可明显缩小肿瘤体积,改善患者一般状况,且未见发生严重不良反应.
BACKGROUND To explore the therapeutic effects of uterine artery interventional chemoembolization on middle and advanced cervical cancer.CASE REPORTS The clinical data of 98 patients with advanced cervical cancer,they were treated with uterine artery interventional chemoembolization from January 2006 to September 2014 were analyzed retrospectively.The volume of postoperative vaginal bleeding and the size of lesion after 3 weeks of the treatment were recorded.Adverse effects,postoperative treatment,tumor residual,lymph node metastasis and 5 year survival rate were observed.The results showed that there were 55 cases with vaginal bleeding of the 98 cases,the effective control rate of bleeding in 72 hours was 100.0%.The results of lesion size after 3 weeks of the treatment showed that the total effective rate of the treatment was 79.6%,no serious adverse reactions occurred during the treatment.After 3 weeks of uterine artery interventional chemoembolization,70.7% of the Ⅰb2-Ⅱa2 phase of patients could receive the radical operation,the rate of lymph node metastasis was 29.2%,the survival rates of 1,3 and 5 years were 98.3%,84.7% and 71.2%.DISCUSSION For patients with middle and advanced cervical cancer,uterine artery interventional chemoembolization could significantly reduce tumor volume,improve the general condition of patients,and no serious adverse reactions occurred.
