摘要
目的观察奥拉西坦联合醒脑静注射液治疗高血压脑出血(HICH)的临床疗效。方法选取2015年12月—2016年12月北京市顺义区医院和首都医科大学附属北京友谊医院收治的HICH患者100例,采用随机数字表法分为对照组与试验组,每组50例。在常规对症治疗基础上,对照组患者予以奥拉西坦治疗,试验组患者予以奥拉西坦联合醒脑静注射液治疗;两组患者均连续治疗21 d。比较两组患者临床疗效,治疗前后美国国立卫生研究院卒中量表(NIHSS)、Barthel指数(BI)评分及血清白介素1β(IL-1β)、血管内皮生长因子(VEGF)、胰岛素生长因子1(IGF1)水平和Tau蛋白含量。结果试验组患者临床疗效优于对照组(P<0.05)。治疗前两组患者NIHSS评分比较,差异无统计学意义(P>0.05);治疗后试验组患者NIHSS评分低于对照组(P<0.05)。治疗前两组患者BI评分比较,差异无统计学意义(P>0.05);治疗后试验组患者BI评分高于对照组(P<0.05)。治疗前两组患者血清IL-1β、VEGF、IGF-1水平及Tau蛋白含量比较,差异无统计学意义(P>0.05);治疗后试验组患者血清IL-1β水平及Tau蛋白含量低于对照组,血清VEGF、IGF-1水平高于对照组(P<0.05)。结论奥拉西坦联合醒脑静注射液治疗HICH的临床疗效确切,可有效改善患者神经功能,提高患者日常生活活动能力,减轻炎性反应并促进脑功能恢复。
Objective To observe the clinical effect of oxiracetam combined with xingnaojing injection on hypertensive intracerebral hemorrhage（ HICH）. Methods From December 2015 to December 2016,a total of 100 patients with HICH were selected in Shunyi District Hospital of Beijing and Beijing Friendship Hospital Affiliated to Capital Medical University,and they were divided control group and test group according to random number table,each of 50 cases. Based on conventional symptomatic treatment,patients in control group received oxiracetam, while patients in test group received oxiracetam combined with xingnaojing injection; both groups continuously treated for 21 days. Clinical effect,NIHSS score,Barthel index,serum levels of IL-1β,VEGF and IGF1,and Tau protein content before and after treatment were compared between the two groups. Results Clinical effect in test group was statistically significantly better than that in control group（ P〈0.05）. No statistically significant differences of NIHSS score was found between the two groups before treatment（ P〉0.05）,while NIHSS score in test group was statistically significantly lower than that in control group after treatment（ P〈0.05）. No statistically significant differences of Barthel index was found between the two groups before treatment（ P〉0.05）,while Barthel index in test group was statistically significantly higher than that in control group after treatment（ P〈0.05）. No statistically significant differences of serum level of IL-1β,VEGF or IGF-1, or Tau protein content was found between the two groups before treatment（ P〉0.05）; after treatment,serum IL-1β level and Tau protein content in test group were statistically significantly lower than those in control group,while serum levels of VEGF and IGF-1 in test group were statistically significantly higher than those in control group（ P〈0.05）.
