无创呼吸机联合纳洛酮治疗老年慢性阻塞性肺疾病合并呼吸衰竭的效果观察 被引量:16

摘要 目的分析无创呼吸机联合纳洛酮治疗老年慢性阻塞性肺疾病(慢阻肺)合并呼吸衰竭的临床效果。方法选取2014年1月至2016年6月本院接诊的72例慢阻肺合并呼吸衰竭老年患者为研究对象,采用随机数表法将其分为对照组和观察组,每组各36例。对照组患者采取常规治疗,观察组患者采取无创呼吸机联合纳洛酮治疗。比较两组患者的治疗效果。结果观察组患者治疗后p H、动脉血氧分压(arterial partial pressure of oxygen,Pa O_2)、动脉血氧饱和度(arterial oxygen saturation,Sa O_2)、第一秒用力呼气量(forced expiratory volume in first second,FEV_1)、肺总量(total lung capacity,TLC)、50%呼气流量峰值(50%peak expiratory flow,PEF_(50)均显著高于治疗前(P<0.05),动脉血二氧化碳分压(arterial partial pressure of carbon dioxide,Pa CO_2)显著低于治疗前(P<0.05)。对照组患者治疗后p H、Sa O_2、TLC、PEF_(50)均显著高于治疗前(P<0.05)。治疗后,观察组患者FEV_1、TLC均显著高于对照组(P<0.05)。观察组患者治疗总有效率显著高于对照组(P<0.05)。结论无创呼吸机联合纳洛酮治疗老年慢阻肺合并呼吸衰竭,能够有效改善患者肺功能,值得临床推广。 ObjectiveTo analyze the effect of non-invasive ventilator combined with Naloxone in the treatment of senile chronic obstructive pulmonary disease (COPD) combined with respiratory failure. Method72 elderly patients with COPD combined with respiratory failure from January 2014 to June 2016 in our hospital were divided into observation group and control group by random digital table method, 36 cases in each group, patients in control group were treated with routine treatment, patients in observation group were treated with non-invasive ventilator combined with Naloxone, the therapeutic efiect of the two groups were compared. ResultAfter treatment, pH, arterial partial pressure of oxygen (PaO2), arterial oxygen saturation (SaO2), forced expiratory volume in first second (FEV1), total lung capacity (TLC), 50% peak expiratory fiow (PEF50) of observation group were significantly higher than those before treatment (P〈0.05), arterial partial pressure of carbon dioxide (PaCO2) was significantly lower than that before treatment (P〈0.05). pH, SaO2, TLC, PEF50 of control group were significantly higher than those before treatment (P〈0.05). After treatment, FEV1 and TLC of observation group were significantly higher than those of control group (P〈0.05). The total efiective rate of observation group was significantly higher than that of control group (P 〈 0.05). ConclusionNon-invasive ventilator combined with Naloxone in the treatment of COPD combined with respiratory failure in elderly patients, can efiectively improve the pulmonary function of patients, and it is worthy of clinical promotion.
